About The Position

The Rehab Liaison acts as a bridge between Breckinridge Health, Inc. and other acute hospitals, inpatient rehab facilities, and long term care facilities to assess patients for subacute rehab suitability, managing the referral process, building relationships with referral staff (nurses, case managers, physicians) to drive admissions, educating families on rehab, and coordinating care transitions, requiring clinical background (Nursing) and sales/marketing skills to meet census goals. Performs on-site evaluation of patients in acute/subacute care settings to determine eligibility for inpatient subacute rehabilitation based on clinical need and insurance criteria. Develops and maintains strong connections with referral sources (physicians, case managers, social workers) through face-to-face interactions and education. Cultivates strong relationships within an assigned territory, prioritizing face-to-face connections. Responds to evaluation orders, review medical records, process insurance/payer information, and facilitate smooth admissions. Educates hospital teams, patients, and families about rehabilitation services, outcomes, and facility programs. Collaborates with hospital teams to ensure timely transfers and manages the overall referral-to-admission process. Utilizes marketing and sales techniques to generate referrals and grow the facility's census. Educates community, referral sources and physicians on our hospital programs and services. Utilizes market analysis to identify new opportunities. Responds to and overcomes admission barriers. Conducts in-services and professional presentations to various groups about our services and outcomes. Assists the Discharge Planner – UR Coordinator with managing insurance authorizations and following up with denials

Requirements

  • Active Kentucky License Practical Nurse Licensure
  • BLS Certification
  • Current driver’s license and acceptable driving record according to company policy.

  • Previous Case Management experience preferred
