The Program Coordinator duties include working with all appropriate members of the health care team to provide coordination of admissions and care through the continuum of services. Ensures that time requirements for completion of Preadmission Screens are met. Ensures that specific patient outcomes are achieved and that variances are evaluated and addressed as necessary. Ensures appropriate utilization of resources and services by the review of admissions, duration of stay, professional services rendered. Consults with system staff (Administration, LSW, RN, MD) as appropriate for complicated issues. Markets services to assigned territory, meeting minimum monthly referral quota as indicated by Program Director. Networking / travel up to 50% of time. Duties also include, but not limited to, unit tours, representation at community outreach events, and physician networking. Requirements

  • Associates degree in related field
  • 1 or more years of clinical experience as an LPN, RN, PT, OT, SLP, RCP
  • Current Clinical license in the state of SC (LPN, RN, PT, OT, SLP, RCP)
  • BLS Required

Nice To Haves

  • Bachelor’s degree preferred
  • Knowledge of Medical terminology
