About The Position

Rehab Liaison Coordinator is a licensed professional who has accountability for the coordination, monitoring and management of Rehab admissions and discharge areas. Acts as a resource to all levels of staff in defining and implementing compliance of all current and new regulatory standards in accordance with TJC, CMS and CARF regulations. Promotes compliance as part of daily operations amongst all team members. Responsible for developing acute rehab census as defined through department targeted goals. This position reports directly to the Director of Rehab Admission. The incumbent may assist and act in a confidential capacity to help manage, formulate, determine and effectuate management process and polices. The incumbent may attend management meetings and may be involved directly or indirectly in the decision making process around confidential management decisions. Position requires exercise of independent judgment.

Requirements

  • Generally requires 3 to 5 years of related experience.
  • Supervisory experience is required.
  • Bachelor's degree in related field is required.
  • A Valid Florida License is required: RN, or Therapist, or Social Work, or any other License in related field.

Responsibilities

  • Leads and executes strategies to maximize timely admissions and effectively collaborate with PFS and referral sources in all levels of care.
  • Recognize, resolve and respond to barriers to admission.
  • Maintains 60% patient assignment workload.
  • Develop referral relationships and collaborate with all internal and external referral sources.
  • Direct supervision of employees' productivity, scheduling, coverage and performance evaluations.
  • Maintain leadership visibility, supporting service excellence initiatives and focusing on improve employee satisfaction.
  • Formulates daily reports for leadership, consult and acute care teams.
  • Review/monitor appropriate medical record documentation compliance for rehab admission.
  • Monitoring 50% and 60/40 CMS Rule on a daily basis.
  • Collaborates with the Rehab team, front line staff, administration and rehab leadership, physicians, advance practice providers as well as the Jackson health system team.
  • In lieu of Directors absence, perform the following tasks: 1) Oversee discharge planning and utilization review for hospitalized rehab patients who require assistance in their transition to their next level of care. Participates effectively in multidisciplinary treatment plan reviews, demonstrating adequate preparation and understanding of patient issues, progress, areas of resistance, positive and negative strategies, etc. Links patient with internal and external resources as needed to meet patient needs. 2) Conduct patient rounds on In patient Rehab units to identify needs and assess patient satisfaction. 3) Utilization Review of collaborative practice to improve quality through coordination of care, length of stay, reducing readmission rates and proper reimbursement. 4) Perform Medical record audits of IRF PAI, IM compliance and discharge documentation. 5) monitoring of timely submission and outcomes of concurrent reviews, denials and appeals.
