A Rehab Director / Physical Therapist (PT / RPT) position is available in a sub-acute/skilled nursing Rehab Department located in Wilton, CT. Requirements

Be a licensed physical therapist or license eligible in the state where employed
Have experience as a treating therapist in the Skilled Nursing Facility setting

Nice To Haves

Experience managing a SNF rehabilitation department

Responsibilities

Ensure efficient and effective operations
Plan and implement rehabilitation programs and procedures to optimize patient outcomes
Monitor patient admissions, schedules, MDS assessment periods and data
Provide administrative rehabilitation supervision
Serve as a rehabilitation staff mentor
Provide physical therapy services as needed
