Position Overview The Rehab Coordinator supports the Complex Rehab team by coordinating patient accounts and serving as a key point of contact for patients, caregivers, referral sources, and healthcare providers. You will help ensure a smooth experience by managing communication, documentation, and insurance-related processes while delivering exceptional customer service with empathy and professionalism. Requirements

• 1-3 years of experience in the home and durable medical industry
• Medicare/Medicaid experience required
• High School Diploma or Technical Degree required (college coursework or degree preferred)
• Strong organizational and communication skills
• Ability to manage multiple tasks and meet deadlines in a fast-paced environment
• Proficiency in Microsoft Office, Outlook, and internet-based applications
• Customer-focused mindset with strong problem-solving skills and attention to detail

Nice To Haves

• Familiarity with medical terminology, anatomy, or rehab services is a plus

Responsibilities

• Coordinate and manage rehab patient accounts from intake through delivery
• Serve as a primary contact for patients, caregivers, clinicians, and referral sources
• Assist with scheduling, intake, insurance verification, and authorization processes
• Prepare and process orders and supporting documentation
• Communicate with insurers to support approvals and confirm coverage requirements
• Collaborate with internal teams to address patient needs and resolve issues in a timely manner
• Support team success by sharing knowledge and contributing to process improvements
• Maintain accurate records and ensure information is entered correctly into internal systems
