Sunnyvale Post Acute is seeking a Rehab Aide to assist in providing treatment and support services for rehabilitation under the direct supervision of a licensed therapist. This role contributes to the overall positive image of the organization and ensures that all required documentation and forms are appropriately completed and included in patient charts. Responsibilities

• Assist with and perform setup as assigned for physical and occupational therapy treatments
• Demonstrate knowledge of treatment goals and precautions while assisting or setting up for patients
• Ensure the safety and cleanliness of the work area and equipment
• Maintain open communication with facility staff
• Confer with therapists to establish treatment schedules
• Transport, transfer, and facilitate patient care as directed by the therapist
• Prepare patient equipment and supplies for treatment
