About The Position

The Rehabilitation Activity Leader at Telecare Corporation is responsible for planning and facilitating rehabilitation services for individuals and groups within a skilled nursing facility. This role focuses on providing psychosocial rehabilitation and skills-building support to older adults with severe mental illness, ensuring a therapeutic environment that promotes recovery and self-help skills.

Requirements

  • One year of direct service experience in a mental health setting.
  • High School diploma or GED equivalent.
  • Valid California Driver's License (Class ‘C') and insurability.
  • Must be at least 18 years of age.
  • CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment.

Nice To Haves

  • AA Degree in a related field.
  • Experience working with adults with serious mental illness and/or adults with developmental disabilities.
  • Knowledge of psychiatric treatment modalities.

Responsibilities

  • Plan and facilitate direct and indirect rehabilitation services for individuals and groups.
  • Demonstrate the Telecare mission, purpose, values, and beliefs in interactions with stakeholders.
  • Implement direct care in accordance with established policies and standards.
  • Establish and maintain therapeutic relationships with members served.
  • Lead or co-lead therapeutic activity groups.
  • Attend shift reports and assist in unit supervision as needed.
  • Maintain inventory and order equipment and supplies for activity programs.
  • Ensure order and cleanliness of the Activity Rooms.
  • Promote and assist members in developing self-help skills.
  • Complete incident reports and notify appropriate agencies of any incidents of abuse.
  • Review and implement service plans for members served.
  • Participate in multidisciplinary team meetings and treatment planning meetings.
  • Demonstrate knowledge of Pro-Act principles to manage aggressive behavior.
  • Observe, record, and report members' social and psychiatric behavior.
  • Assist with restraint of members in the event of assaultive behavior.
