About The Position

Purpose: The Registered Nurse, in coordination with the provider(s), functions as a direct resource for patients coordinating services with other medical facilities and Hospitals for complete care. PART I: ESSENTIAL POSITION FUNCTION AND DUTIES 1. Functions are primary partner with Provider providing direct care to patients. 2. Coordinates education and service delivery to patients internally. 3. In conjunction with the LPN, functions as a lead worker providing oversight for medical service delivery by Medical Assistants at Center. 4. Based on assessment, provide the patient appropriate direct care and education. 5. Refers to or consults with other health providers or resources based on assessment in order to prevent or resolve problems or concerns. 6. Works with the Quality Coordinator auditing and monitoring meaningful uses measures, HEDIS performance and other quality issues. 7. Follows universal precautions. 8. Establishes an interpersonal relationship with individuals or families intended to increase or enhance their capacity for self-caring and coping. 9. Seeks information and generates optional solutions to perceived problems or issues through interactive problem solving with individuals. 10. Participates as a member of a multidisciplinary team to facilitate teamwork, improve patient care processes and outcomes, and maintain an orderly work environment with efficient clinic flow. 11. Responds to crisis situations according to policy. 12. Demonstrates communication skills that are clear, effective, facilitate patient care and team functioning, and is respectful of differences. 13. Supports compliance and Principles of Responsibility, maintains confidentiality, and protects organizational assets. 14. Exhibits ethics and integrity and adheres to applicable federal and state laws and regulations, accreditation and licensing requirements, memorandums of agreements, policies and procedures; reports and/or resolves issues of non-compliance. 15. Assists in the selection, training, and orientation of Medical Assistants. 16. Performs assessments of completeness of care; work cooperatively with providers and health staff to ensure completeness of care. 17. Performs other duties as assigned. PART II: CLINIC WIDE RESPONSIBILITIES 1. Customer Relations: a. Treats guests, patients, physicians, and other employees with care, courtesy, and respect. b. Interacts with other departments on problem issues. f. Accepts feedback from patients, visitors, clinic employees, physicians and general public. 3. Continuous Improvement: a. Continuously looks for and suggests ways to improve. b. Effectively completes assignment to achieve the greatest benefits at acceptable cost. c. Implements improvements as appropriate. d. Demonstrates interest in own growth and development by: i. Periodically evaluating own performance. ii. Demonstrating an awareness of personal abilities and limitations. iii. Independently seeking means to make improvements. e. Attends and participates in in-services and continuing education programs f. Attends departmental meetings. 4. Communications: a. Keeps appropriate people informed. b. Speaks and writes clearly, concisely, and appropriately for need. c. Listens carefully. d. Communicates tactfully. e. Understands that all confidentiality and privacy considerations are respected and fostered at work and off duty. 5. Self –Management: a. Presents a positive image of Western Wayne Family Health Center at all times. b. Carries out assignments with little need for direction. c. Timeliness. d. Maintains confidentiality. e. Provides proper notification of absence and tardiness. f. Works weekends and shifts when necessary.

Requirements

  • Current license as a Registered Nurse required
  • Current Basic Life Support certification required.
  • Knowledge of general nursing principles and practices
  • Knowledge of current nursing technology and methods
  • Skill in the application of nursing techniques
  • Ability to assess the effectiveness of nursing care and to recommend changes to nursing care programs and methods
  • Ability to administer medications and implement treatment plans
  • Ability to instruct others on promotion of health and prevention of disease
  • Ability to provide nursing care direction to medical assistants
  • Ability to maintain composure under stressful conditions
  • Ability to maintain accurate records and prepare reports and correspondence related to the work
  • Ability to maintain favorable public relations
  • Skilled with working in a complex work environment
  • Ability to maintain confidentiality at all times and maintain organizationally appropriate relationships
  • Knowledge of organization policies and procedures.
