About The Position

The Registered Pharmacist at Mount Sinai West is responsible for ensuring rational drug therapy by reviewing inpatient and outpatient medication orders prior to dispensing. The pharmacist evaluates new orders against existing patient profiles and provides appropriate pharmaceutical care.

Requirements

• Bachelor of Science Degree from an accredited college of pharmacy.
• Pharmacist - previous hospital experience preferred but not required.

Nice To Haves

• Certified Pharmacy Technician (CPhT) certification.

Responsibilities

• Demonstrates the ability to detect, communicate and avoid drug interactions and the administration of drugs to which the patient may be allergic.
• Acts as a liaison for prescribers for issue resolution.
• Dispenses prescription medication to inpatients, outpatients and employees as per departmental policies.
• Appropriately supervises non-licensed personnel.
• Checks all work conducted by non-licensed personnel requiring direct pharmacist supervision and authorization.
• Dispenses and maintains accurate records of dispensing controlled substances, tax-free alcohol and investigational new drugs.
• Demonstrates ability to operate required equipment, e.g., unit-dose repackaging, total parenteral nutrition pounder, laminar flow hoods, etc.
• Compounds products as required, including sterile parenteral products, maintaining required logs.
• Initiates and documents pharmacist interventions to change prescribing when required.
• Investigates and documents reports of Adverse Drug Reactions.
• Ensures an adequate inventory of medications and supplies is maintained.
• Arranges for emergency inter-hospital transfer of medications when required.
• Completes all required documentation.
• Maintains a safe, clean working environment.
• Maintains and enforces appropriate security measures at all times.
• Prepares required reports of pharmaceutical services.
• Provides drug information services to other health care professionals.
