About The Position

This is a highly responsible, supervisory public health nursing position, within the Office of Medical and Dental Services (OMDS) of the Okaloosa County Health Department (OCHD). Position will assess, plan, organize, and prioritize assigned clinical systems to improve efficiency, accountability and productivity as related to the HIV/AIDS Ryan White program. Position is required to perform duties necessary to complete projects outlined below for compliance with program goals intended to improve health outcomes. Frequent communication with supervisor is expected and will result in a detailed performance evaluation.   Responsible for the supervision of staff including appraisals, planning and directing work, approving leave, and having the authority for hiring and taking disciplinary action or effectively recommending such actions.   Manages daily clinical operations, medical and non-medical case management operations of the HIV/AIDS Ryan White program per FDOH and HRSA guidelines. Works with supervisor and medical staff to formulate, establish and implement HRSA performance standards and primary care standards of care into clinical practice. Develops systems and processes that assist staff in tracking completion of quality care standards. Utilizes the clinical quality management (CQM) program to assess the extent to which HIV health services are consistent with HHS benchmarks and quality indicators including incorporating strategies to improve delivery of care. In coordination with Executive Community Health Nursing Director, develops policy and or protocols to ensure measures are met. Directs licensed staff and non-licensed staff in daily operations of assigned program tasks and program case management including coordinating specialty medical referrals, tracking progress, and ensuring follow-up or resolution of said referrals. Gathers and assesses data and utilizes data analysis for continuous quality improvement. Involves staff in ongoing quality improvement efforts related to program operations and efficiency. Assures clinic practice methods meet Ryan White program requirements, are evidence based, and incorporate the use of best practices. Conducts programmatic evaluations and writes reports of findings, recommendations; implements, evaluates and monitors corrective actions as needed. Ensures accountability of assigned program applicable drugs, biologicals, vaccines, and compliance of these operational units with all necessary regulations, registrations, and inspections.   Researches and gathers data relating to HIV/AIDS Ryan White quality improvement strategies. Assists supervisor in creating a comprehensive quality improvement or strategic plan according to HRSA & HIVQUAL Program Management guidelines. Plan should include annual goals, objectives, performance measures, staff and community engagement, and other objectives as outlined by HRSA. Works to develop systems for tracking plan goals. Collaborates with supervisor in implementing plan objectives, action steps, and improvement activities. Monitors, tracks and aligns activities with Okaloosa County Health Department Strategic Plan and Ryan White grant objectives on an ongoing basis. Tracks target dates and completion status.   Manages nursing staff in enrolling new clients and providing new client orientation, conducting initial client education and needs assessment at initial enrollment. Instructs and trains staff on strategies to support early linkage and retention in care. Emphasizes program eligibility, program requirements, HIV transmission risk reduction and adherence issues. Manages nursing staff with case management details to ensure clients are retained in care and are receiving needed services. Ensures case management assignments and coding are being accurately maintained and coded in HMS. Assists and mentors program staff on strategies to provide individual-level services for clients.   Manages internal Test and Treat program operations. Responsible for managing the Test and Treat on-call telephone to ensure newly diagnosed clients are linked to care. Assists in the coordination and implementation of all Outreach components within the HIV/AIDS program (Ryan White Program). Assists in the coordination of HIV related special events. As needed, may coordinate with test sites, providers, and community groups to ensure individuals living with HIV receive appropriate services. Responsible for program coordination of all new HIV positive clients, to include coordinating with Area 1 surveillance officer and DIS, and out of state agencies as appropriate. Responsibility includes all follow-up on testing, reporting, and recording lab results, and patient contact for retesting if needed. Assist with annual registration of county HIV test sites as needed.   Assists during clinic as needed. Prepares patients for and assists with examinations. Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s condition. Prepares rooms, equipment, supplies, and assists physician. Discuss cases with physician. Informs physician of patient’s conditions. Instructs clients on topics, such as health education, medication adherence, disease transmission prevention, and develops health improvement plans based on individual client needs. Monitors the quality of care or standards of practice to ensure compliance with state and federal mandates.   Participates in workshops, in services, conferences, meetings and other activities related to public health. Participates in cross training and staff development activities for the purpose of career development. Attends and participates in staff meetings, case reviews and in-services.   Performs all other duties as assigned in support of the Agency’s mission and vision.

Requirements

  • Knowledge of nursing principles and techniques, human anatomy and physiology.
  • Ability to work with patients, to interpret and follow medical instructions, to observe patients’ symptoms and reactions to treatment, to demonstrate manual dexterity and to administer medications.
  • Ability to communicate effectively and to establish and maintain effective working relationships with others.
  • Knowledge of computer functions by using programs, setting up functions, writing software, or otherwise communicating with computer systems.
  • Possesses problem-solving and analytical skills.
  • Communicates effectively in writing and speaking.
  • Knowledge of the principles and processes of strategic planning, resource allocation, leadership techniques and behavioral competencies.
  • The ability to manage and control emotional reactions and conflict in a productive manner
  • Licensure as a Registered Nurse in accordance with Florida Statue 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27
