Requirements

Active Arizona RN or LPN License (or Compact License)
Valid Arizona Class D Driver’s License
Current BLS Certification (online courses not accepted)
Current TB test or Chest X-ray within the last 12 months
Level One Fingerprint Clearance Card
Article 9 Training
Strong time management, communication, and problem-solving skills

Responsibilities

Provide in-home nursing care for low to high acuity patients
Complete patient assessments and follow individualized plans of care
Administer treatments and medications as ordered
Collaborate with families and interdisciplinary team members to achieve positive patient outcomes
Document accurately and communicate patient changes to the RN Clinical Supervisor/Site Director
Safely lift, position, or transfer patients, supplies, and equipment
