Brevard Health Alliance, Brevard County’s only Federally Qualified Health Center, is currently recruiting for a Registered Nurse to join us in the heart of Brevard County’s Space Coast. Since 2005 our focus has been on putting the “community” in Community Health while delivering healthcare to more than 60,000 unique patients annually. The Primary Care nurse is a professional Registered Nurse who provides high-quality, evidence-based care for patients who are cared for in the Primary Care Medical Home setting within a fixed clinical setting or Mobile Van. They perform assessment, triage, case manage, and perform Chronic Care disease management. They must be able to promote a safe environment for all patients including those with altered sensorium due to medication or an acute or chronic illness. Nurses are to document using appropriate forms, follow protocols, and guidelines, adhering to policies and evaluate resources. The RN effectively implements patient activities towards health promotion and disease prevention, assisting the patient in attainment of optimal levels of function through, direct care, as well as provision of supportive measures as appropriate.

  • Uses the nursing process to formulate safe and effective care based on each individual’s unique needs.
  • Maintains clinic nurse and providers schedules to ensure timeliness.
  • Provides comprehensive and coordinated nursing care to scheduled patients.
  • Coordinates, delegates, and directs clinical personnel.
  • Prioritizes care based on patient/family needs, available resources of time, personnel, supplies, and equipment.
  • Documentation is timely, accurate, and concise.
  • Effective communication with patient and family members.
  • Involves patient/family in care decisions and provides education based on identified needs.
  • Effective working relationships with all health care team members, patient, and family members.
  • Protects and respects patients’ rights and confidentiality.
  • Performs daily duties and quality controls including but not limited to medication inventory, point of care testing quality controls, medication refrigerator temperature, stocking supplies, checking for expired supplies and medications.
  • Uses self-directed learning to acquire and maintain current knowledge in nursing practice.
  • Participates in development of clinical goals, standards, protocols, policies, and scheduling.
  • Performs competent care for patients using the primary care medical home model.
  • Triage non-scheduled patients.
  • Track and manage patient’s paperwork.
  • Place referrals and order lab/diagnostic testing according to protocols.
  • RN to cover MA/LPN duties when needed.
  • Call back patients as indicated.
  • Participates with huddles with primary care team.
  • Performs clinic or telephone triage within established standards.
  • Maintains daily patient flow to ensure patients will be seen as soon as possible using all available resources.
  • Enters appropriate patient history into medical record.
  • Provides patient, family and caregiver education based on medical and/or nursing diagnosis.
  • Manages and maintains the delegated critical line.
  • Assists with indigent medication program, as applicable.
  • Enters dispensed medication onto excel spreadsheet and reconciles pharmacy inventory as applicable.
  • Ensures pediatric patients are up to date on all immunizations.
  • Maintains lab Quality assurance (QA) as needed.
  • Documents initial and ongoing patient assessment.
  • Completes patient medical history within established standards.
  • Enters all applicable data into EMR.
  • Takes ownership and represents Brevard Health Alliance’s mission and organizational goals.
  • Graduate from a program accredited by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).
  • Must have an Associate Degree in Nursing, Bachelor of Science in Nursing, preferably from an accredited university.
  • Clear and active Florida/compact Nursing License.
  • Minimum of four (4) years nursing experience with two (2) years recent experience in an outpatient/ambulatory care setting preferred.
  • Current BLS/CPR certification from the American Heart Association or the American Red Cross.
  • Proficient in Electronic Medical Records (EMR).
  • Demonstrates strong oral and written communication skills.
  • Experience in a primary care medical home model.
