Requirements
- Active Texas RN license
- 1+ year clinical experience (home health preferred)
- Current CPR
- Reliable transportation
- Strong documentation and communication skills
- Professional, compassionate, patient-focused care approach
Responsibilities
- Complete skilled nursing visits and assessments
- Manage wound care, medication teaching, and patient evaluations
- Coordinate care plans with physicians and clinical teams
- Maintain accurate and timely documentation
