We are looking for a Registered Nurse (RN) for our team in Missouri. This is a field-based position that will include fieldwork throughout the Kansas City area. Department & Position Overview: Personal Care Services is a caregiving arrangement in which a patient has a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in the patient’s home that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient. In this position, as a licensed practical nurse, you will perform clinical evaluations in the home of the Personal Care Services member to determine the level of service to qualify for the program. You will also conduct monthly visits with members who are On Care with FreedomCare to ensure they are receiving quality service and address any changes in their condition.

Requirements

  • Licensed Registered Nurse licensed by Missouri Board of Nursing.
  • Minimum of 2 years clinical experience, 1 year of experience with elders or adults with disabilities
  • Valid driver’s license with an insured vehicle for travel within the state
  • Strong communication and interpersonal skills.
  • Problem-solving and decision-making abilities.
  • Organizational and multitasking skills.
  • Excellent communication, critical thinking, and organizational skills
  • Self-motivated
  • Able to thrive in a fast-paced start-up environment

Nice To Haves

  • Bilingual in Spanish is a plus
  • Experience with CRM (Salesforce is a plus)
  • Home health or home community-based services experience
  • Start-up experience
  • Understanding of regulatory and provider compliance requirements
  • Policy development and implementation

Responsibilities

  • Respects client and employee rights and privacy, ensure the security of protected information, practice in an ethical manner and is compliant to the agency’s compliance program and privacy policy.
  • Follows theFreedomCare’sclinical policy and procedures.
  • Regularly reassesses(home visits) the client’s needs to ensure appropriate plan of care is in place. Properly completes PCCP referral to State to request modifications to plan of care.
  • Oversee the completion of caregiver pre-hire and annual training requirements. Approves caregiver’s competency to complete plan of care tasks (home visits)
  • Initial and on-going instruction of care to Caregiver according to the plan of care (home visits).
  • Completes all client documentation, to include written records, reports, and notesin accordance withagency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
  • Actively coordinates the care of the client through communication and written documentation to the physician,RNand Program Director, other disciplines involved in the client’s care and community resources.
  • Interprets the philosophy andobjectivesof the agency to clients, families, and others.
  • Performs other job-related duties as mayberequired or directed by the supervisor.
