About The Position

The Registered Nurse (RN) in the Skilled Nursing Facility is responsible for providing high-quality clinical nursing care to residents. This role encompasses comprehensive assessment, planning, implementation, and evaluation of nursing care tailored to individual patient needs, consistent with the physician's plan of care and facility policies. The RN serves as a supervisory leader for Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs), ensuring that residents receive continuous, compassionate care throughout their stay. They act as patient advocates and educators, guiding residents and families through the complexities of aging and specific health issues.

Requirements

High School Diploma or equivalent to (REQUIRED).
Current California RN license
Basic Life Support (BLS) Certificate
Ability to communicate clearly in both oral and written forms using the English language.
Ability to respond to common inquiries or complaints from residents, patients, physicians, peers, regulatory agencies, or members of the business community.
Ability to work independently.
Leadership Skills: Demonstrated ability to supervise and lead a team, coordinating care effectively among various healthcare providers.
Clinical Knowledge: Proficient knowledge of nursing practices, principles, and regulatory compliance governing long-term care.
Emotional Intelligence: Maintain a positive and patient demeanor, managing challenging situations with grace.
Commitment to Continuous Learning: Willingness to apply new methods and practices to enhance resident care.
Ability to lift, move, or transfer residents as necessary, including assistance with mobility; with the ability to lift to 50 pounds (or more when using assistive devices) occasionally assist with heavier patients.
Capable of pushing or pulling equipment, such as stretchers and wheelchairs, weighing up to 100 pounds.
Capable of standing, walking, and physically supporting residents throughout shifts.
Adept at using medical equipment and technology in a clinical environment.
Ability to stand, walk, and move quickly for extended periods.
Ability to perform CPR and manage emergency situations.
Dexterity for tasks like injections and operating medical equipment

Nice To Haves

Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
A minimum of 2 years of clinical experience in a patient care setting is preferred.

Responsibilities

Patient Care and Advocacy: Provide direct clinical care and serve as an advocate for residents, ensuring that care aligns with each patient’s preferences and care plans.
Care Coordination: Collaborate with physicians to develop and implement individualized patient care plans; notify physicians promptly of any changes in residents’ conditions.
Education and Communication: Educate residents and family members on managing medical conditions, recovery strategies, and long-term wellness goals. Communicate significant changes in resident conditions and treatments to their families.
Supervision and Delegation: Supervise CNAs and LPNs, ensuring compliance with established protocols and high standards of care. Delegate appropriate nursing tasks.
Operational Standards: Ensure compliance with safety and sanitation standards in all work areas and residents’ rooms.
Medication Management: Order and manage medications, equipment, and supplies as prescribed; prepare and administer medications in accordance with regulations and procedures.
Clinical Assessment and Monitoring: Conduct continuous assessments of resident health, monitoring vital signs and responses to treatment; document all observations accurately.
Documentation: Complete charting and documentation following established protocols, ensuring accuracy and timeliness.
Professional Nursing Services: Perform tasks such as tube feedings, catheterization, suctioning, blood draws, and dressing changes as needed.
Collect specimens for lab testing and ensure accurate results are communicated to appropriate personnel.
Assist with the orientation of new staff, fostering a collaborative work environment.
Collaboration with Healthcare Teams: Attend medical rounds with staff physicians, nurse practitioners, and physician assistants to ensure coordinated care.
