Requirements
- Graduate of an accredited nursing program (BSN preferred).
- Current Arizona Registered Nurse (RN) license in good standing.
- At least 1 year of professional nursing experience (home health nursing experience strongly preferred).
- Strong clinical judgment, communication, and organizational skills.
- Valid driver’s license, reliable transportation, and ability to travel to patient homes in Sun City and surrounding Maricopa areas.
Nice To Haves
- Public Health Nurse (PHN) certification a plus.
Responsibilities
- Provide skilled nursing care in the patient’s home setting, including assessments, treatments, wound care, and medication management.
- Develop and implement individualized home health care plans, adjusting as patients’ conditions change.
- Educate patients and families on health conditions, treatments, and at-home care.
- Collaborate with the Director of Nursing, physicians, and interdisciplinary teams to coordinate quality care.
