This is a full-time position, 40 hours per week, working with the Beth Israel Lahey Primary Care (521 Mount Auburn St, Ste 103, Watertown, MA). 1. Coordinate and facilitate quality clinical management of a broad based outpatient population across the continuum of care. Collaborates with providers to identify needs for preventive care, evidence-based chronic disease management and care coordination. Provides general and specific healthcare education. Works directly with patients to identify and monitor self-management goals. 6. Collaborates care with community based providers. Follows up on ED visits and inpatient discharges. Assists providers with exams, treatments and procedures as needed. 7. Provides skilled telephonic triage and assessment, education, collaboration and coordination of healthcare and community resources. Performs a direct patient care role. Nursing visits include : Annual Wellness Visits (AWVs) and Chronic Care Management (CCM) of our patients. 8. Responsible for INR management and teaching for those patients on Coumadin using a standardized protocol. 9. Administers vaccines and medications by parenteral, oral, rectal or nasal route by orders of the prescribing provider and by the Medication Administration policy guidelines established. Provides patient education as appropriate for the administered medication. 10. Performs a wide range of nursing activities as needed under the direction of the provider such as wound care, point of care testing etc. Accurately documents all aspects of patient care in the Electronic Health Record in a timely basis. Schedules tests and referrals for specialty care on order of provider. 11. Monitors and ensures follow up of all abnormal test and imaging results under the direction of the provider. 12. Oversees the inventory and ordering management of clinical supplies and equipment as necessary. Ensures safe, accurate and clinically appropriate collection, storage and processing of all biologic specimens. 13. Provides oversight, delegation and acts as a resource to unlicensed staff including MAs, Certified Nursing Assistants and Nursing Students. Provides on-going education and training to practice staff. Updates clinical knowledge and skills though consultation, review of literature and formal and informal education including CEUs. Maintains compliance with organizational mandatory annual training. 14. The Team Nurse will ensure that PHI (Patient Health Information) will only be used for the purposes of patient TPO (treatment, payment and operations). The Team Nurse will ensure patient safety practices to prevent/eliminate risk of patient harm. Adverse patient events and near misses will be promptly reported to the Risk Management Dept. Job Description: Principal Duties and Responsibilities 1. Act in compliance with nursing philosophy, practice standards, and qualifications. 2. Collaborates with physicians and midlevel providers in providing direct healthcare to patients. Evaluates client outcomes and condition changes. 3. Communicates and documents teaching and follow-up care with patients. Documents patients’ understanding of plan/instructions. 4. Communicates pertinent patient information and findings to providers, associate nursing peers, and other caregivers in a timely manner. 5. Triages telephone inquiries, reviews lab results and diagnosis testing, and assures written patient notification of test results, missed appointments, and abnormal study follow-up when appropriate. Arranges referrals to ancillary services. 6. Demonstrates competency in office including testing, phlebotomy, and assists physician/midlevel provider with procedures. 7. Maintains and supervises the integrity of departmental logs. 8. Reviews and comprehends Department of Public Heath and Welfare information and recommendations for prophylaxis, treatment, and reporting of communicable diseases. Completes informational forms such as premarital and WIC. Recognizes signs and symptoms of child and domestic abuse and reports accurately. 9. Maintains stock of client education materials. Reviews new material available. Organizational Responsibilities 1. Follows hospital and departmental policies with special attention to safety, dress, attendance and punctuality, and display of ID badge. Subscribes to hospital’s mission statement, code of ethics, and standard of conduct. 2. Handles all work-related information in a confidential manner, as outlined in Confidentiality Policy B.91. Recognizes and respects each patient’s rights to privacy, and understands that this is protected by statue. Understands that looking up patient information not needed for job duties is prohibited and is grounds for immediate dismissal. 3. Treats customers with courtesy and compassion and responds to requests in a timely manner. 4. Works effectively with members of the team so that they meet constantly changing needs and deliver care that is customer oriented and cost effective. 5. Demonstrates initiative and creativity to continuously improve services, departmental and interdepartmental processes, and any other activities that affect quality. 6. Plans organizes and renders care for neonate, pediatric, adolescent, and geriatric patients within an assigned area 7. Performs work responsibilities while staying in compliance with all applicable laws, rules, and regulations. Understands and agrees to uphold the Hospital Compliance Plan. Patient Safety 1. When taking a telephone order, writes the order on the Doctor’s Orders, reads the order back to the MD and document as a TORB. 2. When taking a verbal order, repeats the order back to the MD prior to implementing the order and documents as a VORB. 3. Uses only accepted abbreviations when documenting in the medical record. 4. Uses two unique patient identifiers prior to treatment or patient transport. 5. Consistently uses sharps devices as designed to prevent injury. 6. Complies with Isolation Guidelines; wears appropriate personal protective equipment as required by specific patient precautions. 7. Consistently complies with CDC Handwashing Guidelines. Cleans hands before and after patient contact, or contact with patient environment.
