About The Position

All Heart Home Health is seeking a compassionate and dependable Full-Time Registered Nurse (RN) – Field Nurse to provide high-quality, patient-centered care in the Nashville, TN area. This role is ideal for a nurse who enjoys working independently while making a meaningful impact in patients' homes.

Requirements

Active Registered Nurse (RN) license in the state of Tennessee
Graduate of an accredited nursing program
Minimum 1 year of nursing experience (home health experience preferred)
Strong clinical assessment and critical-thinking skills
Excellent communication and time-management abilities
Ability to work independently in a field-based role
Valid driver's license and reliable transportation
Current CPR/BLS certification
Ability to pass background check and drug screening

Responsibilities

Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans
Perform comprehensive patient assessments and ongoing evaluations
Administer medications, treatments, injections, and wound care as ordered
Educate patients and families on diagnoses, medications, and care plans
Accurately document all patient care and progress in the electronic medical record
Collaborate with physicians, case managers, and interdisciplinary team members
Monitor patient condition and report changes promptly
Ensure compliance with state, federal, and All Heart Home Health policies and procedures
Participate in on-call rotation as assigned (if applicable)
