About The Position
We are seeking a Registered Nurse for our House Calls program. This is an excellent opportunity for an experienced and passionate medical professional to make a meaningful difference in the lives of our patients by providing them with high -quality, compassionate, and comprehensive health care in the comfort of their own homes. The ideal candidate for this position should be a highly motivated and organized individual with an advanced degree in nursing and experience in a clinical setting as an RN. The Nurse will be part of our growing medical professionals. Your primary responsibility will be to provide advanced primary and/or specialty care to our patients.
Requirements
- Current New York State License as an RN
- National Board Certification required
- American Association of Nurse Practitioners (AANP)
- American Nurses Credentialing Center (ANCC)
- New Grads are Encouraged to Apply
Nice To Haves
- Bilingual (Spanish Speaking) is a plus
- EMR/EHR experience a plus
Responsibilities
- Conduct in -home assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
- Collects, records, and maintains accurate patient information
- Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
- Perform evidence -based practice screenings including point -of -care testing (as appropriate)
- Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
- Communicate findings in your assessment that will be used to inform member’s PCP of potential gaps in care
- Identify urgent and emergent situations and intervene appropriately
- Educate members on topics such as disease process, medication, and compliance
- Documents items such as past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan
- Orders or executes various tests, analyses, and diagnostic images to provide information on a patient's condition
- Ensures fulfillment of our commitment for ensuring quality care
- Patient -centered care in collaboration with other health care providers, and clinical staff to identify and address gaps in care.
