About The Position
Registered Nurse (Clinic-based) Full-time 7:30 a.m. - 4:00 p.m. $75k - $82k Responsibilities Include: Conducts assessments for new enrollees and periodic reassessments for current participants. Coordinate with Home Care Coordinator to ensure CSC participants receive appropriate home care. Contributes to the development and revision of participant care plans. Completes Nursing documentation and ensures completion, accuracy and relationship to priorities and goals. Participates in providing 24/7 'on call' coverage in collaboration with other professional staff, as assigned. Attend CSC team meetings, care plan meetings, and including scheduled meetings with participants and/or families. Education, Training And Experience: Registered Nurse and be a graduate of a school of professional nursing. Certified in CPR Three years’ experience in nursing Minimum one year nursing experience with home care, geriatrics, case management, or discharge planning with frail elderly population in acute long-term care, or community-based setting. In good standing with the Medicare and Medicaid programs
Requirements
- Registered Nurse and be a graduate of a school of professional nursing.
- Certified in CPR
- Three years’ experience in nursing
- Minimum one year nursing experience with home care, geriatrics, case management, or discharge planning with frail elderly population in acute long-term care, or community-based setting.
