About The Position

ChristianaCare is currently seeking a full-time Registered Nurse for the Community Health Impact, Community Medicine at the Hope Center located at 365 Airport Road, New Castle, DE. Work Schedule Mon-Fri, day shift with some rotation to late day coverage until 8:00PM. 40 hours per week Primary Duties and Responsibilities Deliver age-appropriate care to adult and geriatric patients in both virtual and on-site settings, applying knowledge of growth and development and patient assessment. Manage cases for medical respite and primary care patients, including coordination of admissions, discharges, and referrals from internal and external sources (e.g., inpatient units, ED, mobile van, CareVio). Collaborates with interdisciplinary teams to ensure seamless transitions of care across settings (e.g., hospital to home, specialty referrals). Conduct nurse visits such as Annual Wellness Visits, Advance Care Planning, Transitional Care Management, and chronic disease management. Perform clinical tasks including point-of-care testing, EKGs, medication administration, wound care, and routine procedures as directed. Triage walk-in patients, ensuring appropriate care and disposition. Respond to patient crises and emergency situations. Provide patient education on wellness, self-care, and medication adherence. Facilitate follow-up appointments, diagnostic testing, imaging, and DME orders. Collaborate with the county team on admission processes. Serve as liaison between patients, providers, and staff to ensure clear communication and efficient patient flow. Support LPNs and MAs in clinical duties and care coordination. Participate in infection control, quality assurance, and compliance initiatives. Maintain current knowledge of policies and procedures; engage in continuous quality improvement. Support JCAHO readiness, POCT compliance, and Safe Practice Behavior Monitoring (SPBM). Assist with onboarding and competency evaluation of new and existing staff (LPNs and MAs). Conduct chart audits and shadow clinical support staff to ensure quality care. Contribute to staff education and training initiatives. Respond to telephone inquiries under provider direction. Ensure adequate staffing coverage as needed. Participate in special projects and maintain a safe work environment by adhering to safety protocols and reporting hazards. Performs other related duties as required.

Requirements

  • A graduate of a state approved or National League of Nursing accredited School of Nursing
  • RN license or eligible for licensure to practice in the state Delaware
  • Two years’ experience as a RN, physician office experience preferred
  • BLS certification is required.

  • BSN preferred; ADN considered with agreement of completion of BSN in 3 years
