This position plays a pivotal role in maintaining top tier patient care. Requirements

Must have a nursing degree from an accredited program
Licensed as a registered nurse within the state of Florida
Adherence to federal, state, and accreditation requirements, including Medicare and Medicaid regulations.
CPR certification required (Online certification not accepted).
At least one year of experience at a home health agency or within a hospital.

Responsibilities

Coordinate patient care in accordance with the treatment plan created by the physician.
Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
Participate in the patient's discharge planning process.
Prepare progress notes following visits for patients’ charts.
