Registered Nurse - Long Term Care/SNF
About The Position
We are seeking an experienced RN for an 8-hour night shift assignment in a 34-bed LTC/SNF unit situated within an acute care setting. This unit provides long-term care and skilled rehabilitation services to patients ranging from ages 45–99+. The assignment is available for up to 32 weeks, with every other weekend required. Floating is required as needed (no call), including potential support in the acute hospital setting as a sitter. The unit utilizes Sigmacare and Epic for documentation and medication management, with CNA/PCT support and an LTC Nurse Practitioner assigned to the unit. Respiratory services are available 24/7. No block scheduling permitted, and reliable transportation is required as public transportation is not accessible in the area.
Requirements
- Minimum of one year of RN experience, including LTC/SNF experience
- Active NY RN license (or proof of application)
- BLS certification
- Willing to float as needed
- Work 8-hour night shifts
- Commit to every other weekend
- Explanation for all employment gaps over 90 days
- Complete work history details (including bed count, EMR systems, and facility type)
