As a Birch Family Services Registered Nurse, you will be responsible for the assessment, planning, implementation, and supervision of medical care for individuals with intellectual and developmental disabilities in a system of residential programs. Collaborating with clinical and ancillary staff, you will promote health and safety and provide case management services. Review assessments, logs, etc. and follow-up where necessary; ensure all medical appointments, tests and lab work are completed as required; and assist with the AMAP certification process. Requirements

  • Associate’s or bachelor’s degree in nursing (ASN or BSN) required
  • NY State RN License in good standing
  • 1-2 years clinical experience, preferably in an OPWDD or mental health setting required.
  • Sufficient computer skills to operate email, Microsoft Office Suite, and electronic health records platforms required.
  • Regularly required to speak clearly and hear the spoken word well.
  • Regularly required to utilize near vision ability to read data and documents, including spreadsheets and reports, in printed form and on computer screens.
  • Regularly required to provide physical care for clients and individuals, which may include, but is not limited to bathing, feeding, changing clothes or diapers, and pushing wheelchairs.
  • Ability to physically restrain challenging behaviors that include aggression, self-injurious, and destructive behaviors.
  • Ability to apply proper lifting techniques.

Responsibilities

  • Provide and supervise comprehensive nursing services and interventions to ensure that the health care needs of individuals receiving services are met.
  • Provide continuity of medical care and services to individuals by monitoring the planning, organizing, directing, and controlling nursing activities including those carried out by support staff.
  • Assure that the medical needs of the individuals are addressed by working with physician(s) as appropriate.
  • Follow through with the provision of nursing care by monitoring nursing and related services.
  • Develop a Nursing Care Plan for each individual and ensure the plan is reviewed and revised as needed.
  • Provide for the individual’s continued health, welfare and safety by direction, coordination, and supervising the individual’s medical care plan along with the implementation of all agency and OPWDD policies and procedures that are related to the Rights, Care, Treatment, and Protection of all individuals.
  • Identify, assess and report circumstances occurring at the time a person experiences a seizure, injury, illness, behavioral episode, and/or any other medical or emotional difficulty an individual may have and implements therapeutic interventions accordingly.
  • Participate in Treatment Team including annual, semi-annual and weekly medical meetings; both off and on site.
  • Collaborate with the Residence Manager to ensure that all staff are medically trained to meet the individuals’ needs by monitoring, training and evaluating all med-certified staff as per Birch and OPWDD guidelines.
  • Conduct delegated nursing training for program staff, as well as general health/hygiene, infectious disease control/prevention, and other related topics for agency staff development.
  • Update MARS monthly and as needed.
