About The Position

The Registered Nurse (RN) at Treasure Coast Hospice is responsible for the planning and delivery of patient care, providing skilled nursing care and oversight in accordance with Treasure Coast Hospice policies and the Florida Nurse Practice Act. This role utilizes the nursing process of assessment, planning, intervention, implementation, and evaluation to provide palliative and supportive care to patients and their families. The position includes coverage for Clinical Support Services, such as phone triage (call center), in-person visits (responder), crisis care, and providing coverage to inpatient units as needed. The RN will provide care across Martin, St Lucie, and Okeechobee counties.

Requirements

  • Current and active Florida RN license.
  • Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.
  • Current and valid Florida driver’s license and automobile insurance.
  • Minimum of one (1) year RN experience with acute care experience.
  • Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
  • Must be able to effectively plan, organize and prioritize work.
  • Must be self-directed and able to work independently in the field.
  • Ability to read, write and speak English.
  • Excellent communication and interpersonal skills.
  • Must hold a current (BLS) CPR certification.
  • Attendance is a condition of employment.

Nice To Haves

  • Acute care experience, preferably in ER or as a Charge Nurse.
  • Prior experience in oncology, home health, Hospice or palliative care setting.
  • Bilingual preferred.
  • Basic computer skills and EMR software experience preferred.

Responsibilities

  • Provide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments.
  • Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development.

  • Be current in legal and compliance hospice issues.
  • Meet or exceed identifiable standards for the position.
  • Uphold Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.
  • Provide comprehensive information to callers, and assist with emergent visits when needed.
  • Initiate/cover crisis care cases, case management coverage visits, transition alert visits, assistance with admissions, crisis care planning and any other visit types deemed necessary for the safe, effective provision of patient care as needed including back up staffing to the IPU and CNA visits depending on census and TCH care needs.
  • Clearly understand and adhere to each Standard.

  • Perform comprehensive assessments to include the physical, emotional, spiritual and psychosocial needs of the patients and family unit.
  • Communicate to the IDT all pertinent information pertaining to the patient and family unit, actively to participate in review and revision of patients’ plan of care.
  • Document variance in patient condition, interventions and outcomes and address progress towards goals set by the patient’s plan of care.
  • Based on verbal assessment determine the needs of the patient and family member.
  • Assign (call center/triage) staff based on the need.
  • Possess knowledge of commonly used medications used with the Hospice patient.
  • Document education provided to the family and the patient, including topic, which was taught and their understanding of the education.
  • Make patient visits to wherever the patient calls home; as indicated by need.
  • Initiate communication with physicians and other staff members to coordinate optimal care and use of resources for the patient/family.
  • Teach appropriate interventions, medications, effects and side effects to patient, caregiver, volunteers and others as appropriate.
  • Support, instruct and educate the patient, family, and caregiver.
  • Anticipate, prevent and treat undesirable symptoms or secondary symptoms.
  • Assist in determining the need to change the level of care being provided for hospice patients based on assessment and collaboration with attending physician and/or hospice medical director.
  • Provide appropriate support at the time of death, including disposition of medications, notification to funeral home, etc. according to agency policy.
  • Maintain documentation in the EMR relating to care coordination including phone calls, communication, visits, and other pertinent clinical information.
  • Participate as an active supporter of the Treasure Coast Hospice compliance program, including an emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization’s compliance program.
  • Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.
  • Perform other duties as required and conform with and abide by all policies and procedures.
  • Provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals, and maintain good standing with each.
  • Provide direction to RN and LPN as needed.
  • Provide direction to CNAs on an ongoing basis.
