About The Position

The Elevated Community Health (ECH) Registered Nurse will promote, maintain, and improve individual and community health by supporting patients in adopting healthy behaviors. The Registered Nurse will provide safe, effective, and high-quality primary, preventive, and evidence-based healthcare to patients in a culturally competent manner, focusing on health promotion and disease prevention. The Registered Nurse is responsible for assisting medical providers and medical assistants in the efficient and orderly delivery of medical services and performing related duties as assigned by the Chief Medical Officer. The Registered Nurse acts as a resource to the medical assistants, front desk staff, health educators, and providers. The Registered Nurse must demonstrate respect for the rights of all patients and provide respectful and considerate care that is focused on the patient's needs. The Registered Nurse will provide patients, families, and personnel with a safe and confidential environment.

Requirements

  • A Bachelor of Science in Nursing (BSN) degree or an advanced degree in Nursing is preferred. Associate degree considered
  • Current Licensure as a Registered Nurse from the State of Colorado.
  • Current CPR card.
  • At least two years of experience in a clinical setting will be considered for new graduates on a case-by-case basis.
  • Ability to read, write, and speak the English language.
  • Ability to interpret various instructions and deal with multiple linguistic and cultural variables.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to exercise independent judgment.
  • Ability to reason objectively.
  • Ability to assess, project, and plan for patients' needs.
  • Ability to interpret state/federal/agency regulations.
  • Ability to document concisely, accurately, and in a timely manner.
  • Ability to handle various duties that may be interrupted and/or changed by immediate circumstances.
  • Ability to relate cooperatively and constructively with patients, coworkers, administration, health care providers, community agencies, referral sources, regulators, and other health team members.
  • Ability to enlist the cooperation of others.
  • Ability to function well in a high-stress environment.
  • Competent knowledge of Microsoft (Word & Excel) and database software.

  • Previous non-profit and/or volunteer experience preferred.
  • Ability to speak the Spanish language (bilingual) preferred.

Responsibilities

  • Deliver quality service, creating solutions that add value to a patient's health.
  • Provides clinical support to medical assistants, community resource coordinators, front desk staff, and providers.
  • Provide high-quality patient triage over the phone or in person.
  • Provide case management services for high-risk patients who require additional support in navigating the healthcare system.
  • Provide direct patient care in the context of nurse visits and co-visits (also known as flip visits) with medical providers under the supervision of the designated medical provider.
  • Accurately document patient interactions/information in the patient's electronic health record.
  • Complete paperwork regarding vital signs, allergies, etc.
  • Accurately chart lab results and send reminder notices and other notes in a timely manner.
  • Act as a patient liaison for prior authorization requests from pharmaceutical companies and pharmacies to allow patients to obtain necessary medications or services.
  • Focus on quality customer service for all patients, coworkers, volunteers, and partners outside ECH.
  • Participate in the Clinic's QI/QA Program as directed by the supervisor. This may include the collection, reporting, and/or review of quality data, as well as participation in performance improvement projects.
  • Provides clinical and non-clinical in-services for ECH staff.
  • Support and document patient phone messages, including processing prescription refills per protocol.
  • Assist medical providers with patient procedures and testing as needed.
  • Assist the Chief Medical Officer with projects as assigned.
  • Maintain compliance with Occupational Safety and Health Administration regulations. Responsible for maintaining knowledge of these regulations and applying guidelines in the workplace.
  • Maintain all aspects of patient confidentiality in accordance with HIPAA regulations.
  • Refers to and consults with other health care professionals as needed.
  • Educates patients and their family members about healthcare issues.
  • Attends staff meetings.
  • Actively participates as a member of the integrated health care team, working in conjunction with the Care Team and Dental and Behavioral Health departments to ensure seamless, multidisciplinary patient care.

  • Complete all required training as designated by HR and the position supervisor.
  • Adhere to all organizational policies and those outlined in the ECH Employee Handbook.

  • Willingness to travel and work at other ECH locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements in a timely manner.
