The PACE Primary RN promotes and maintains participants’ health in the community through education, counseling, and coordination of preventive and rehabilitative services with the interdisciplinary team. The RN assesses health needs, oversees and implements nursing care plans, and provides direct nursing care. The role includes ongoing re-evaluation of individual and family needs. The Primary RN coordinates scheduling, communication, and documentation to ensure efficient home care services. The RN also oversees and updates the CNA plan of care. What is PACE? PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states. The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise. You Bring: Graduated from an accredited school of nursing is required. Bachelor’s degree in Nursing is preferred.
