The successful candidate will be licensed in good standing as a Registered Nurse (RN) in the State of Louisiana and will be able to contribute to the provision of quality, comprehensive medical care and support to the CHNO residents and staff, respectively. The ability to communicate with a diverse patient population is required. Direct and coordinate trauma-informed, health/medical related activities to ensure that the medical needs of youth and residents experiencing homelessness are met and comply with Federal, State, and Local Regulatory agencies. The RN leads on medication oversight, management, staff training, education, and support, liaison with onsite medical clinic and community pharmacies, as well as assisting youth in assessing their medical needs and accessing necessary referrals.

  • Initiate and maintain positive, professional, and mentoring relationships with youth and staff in accordance with the CHNO Mission, Vision, and Core Values.
  • Promote a peaceful, healing environment by utilizing the principles and practices of trauma-informed care and positive youth development in all aspects of work and youth engagement, including crisis prevention, intervention, and de-escalation.
  • Ensure that CHNO is a welcoming, safe, and secure “homelike” environment for youth receiving services.
  • Assess current policies, procedures, systems, and protocols in place for resident medical experience from intake, initial medical assessment, wellness/mental health assessment, and medication management, and make recommendations where applicable and/or needed for compliance purposes.
  • Draft and maintain Standard Operating Procedures (SOPs) that outline the systematized flow of resident access to initial, follow-up, and ongoing medical care appointments on and off CHNO site.
  • Create and facilitate training materials and workshops for frontline CHNO staff including but not limited to case managers, resident advisors (RAs), and other staff as needed.
  • Assess and monitor program delivery to adherence to SOPs and make recommendations for improvement as needed.
  • Establish SOPs for optimal medication adherence among residents from initial prescription, pick up/delivery to campus, individualized distribution schedules, and refill follow-up.
  • Train staff to ensure proper labeling, storage, and tracking of resident prescriptions.
  • Establish system of and train staff on managing resident prescription disbursement schedules that meet the individual needs of each resident.
  • Establish and maintain SOPs for medication handling that includes but is not limited to medication tracking from initial prescription, pick up/delivery to campus, distribution schedules on site, refill follow documentation, medication checklist for transitioning residents, to disposal of unclaimed/expired medications from transitioned residents.
  • Create care coordination flowchart of steps for residents from intake through initial contact with on-site Tulane Adolescent clinic to timely completion of mental health referrals and off-site specialist referrals
  • Recommend and collaborate with Program leadership to develop health education, prevention, and risk reduction programs for youth as related to trends associated with overall youth health and wellness.
  • Establish enhanced campus wide approach to educating residents on health prevention across all residential spaces and programs.
  • Serve as a liaison between on-site Tulane Adolescent Clinic and CHNO staff to ensure smooth communication feedback loops are in place.
  • Work with program staff to review and update our medication administration policy, provide training to staff, and implement a Quality Assurance (QA) review of medication administration.
  • Provide consultation and technical assistance to CHNO staff regarding health issues.
  • Ensure appropriate training requirements and employee health assessments and screenings are performed, documented, and maintained for employees according to policy and regulatory agency policy.
  • Ensure accurate and mandated records on all patients are maintained and are kept confidential.
  • Ensures compliance with all regulatory agencies to maintain licensure and adhere to all standards.

  • Bachelor of Science Nursing (BSN)Degree
  • Current nationally Board-Certified RN licensure in State of Louisiana required.
  • Educational instruction for staff and youth.
  • Program development, implementation, and evaluation.
  • Clear understanding of the Medical Model.
  • Strong knowledge of congregant living health risks and mitigation strategies, epidemiology, communicable diseases and patient assessments, preferred.
