Requirements

• Active Virginia RN license (required)
• 3+ years nursing experience
• Knowledge of Medicare home health regulations
• OASIS documentation experience
• Valid driver’s license and insured vehicle
• CPR certification

Nice To Haves

• Prior home health RN experience
• Experience with WellSky (Kinnser) EMR
• Strong case management skills

Responsibilities

• Conduct skilled nursing home visits and patient assessments
• Lead Start of Care, ROC, and discharge visits (OASIS)
• Deliver wound care, medication management, IV therapy, teaching
• Manage your patient panel with RN case ownership
• Coordinate care with LPNs, PT/OT, and office team
• Chart visits in WellSky (Kinnser) within required timelines
• Escalate patient condition changes quickly
