About The Position

Reliable Home Care Providers, Inc. is hiring a Registered Nurse (RN) to provide skilled home health nursing services to patients throughout the North Side of Chicago. This part-time, field-based position is ideal for an RN seeking scheduling flexibility, professional autonomy, and meaningful patient care in the home setting. The RN will manage patients across the full episode of care, from admission through discharge, while collaborating closely with physicians, caregivers, and the interdisciplinary care team.

Requirements

  • Current Registered Nurse (RN) license in the State of Illinois
  • CPR certification
  • Prior nursing experience in home health, acute care, or other clinical settings
  • Strong clinical judgment, problem-solving, and critical-thinking skills
  • Ability to work independently while managing a dynamic caseload
  • Strong communication and patient engagement skills
  • Valid driver’s license, reliable transportation, and active auto insurance

Nice To Haves

  • Home health nursing experience
  • Familiarity with electronic medical record systems, including WellSky Home Health
  • Experience supervising clinical staff

Responsibilities

  • Complete the nursing process in the delivery of patient care, including assessment, planning, implementation, and evaluation
  • Maintain overall clinical oversight of patients from admission through discharge
  • Develop and update individualized plans of care based on comprehensive assessments
  • Serve as a patient advocate, maintaining ongoing communication with physicians, referral sources, caregivers, and facility partners
  • Ensure timely, accurate, and compliant skilled nursing documentation
  • Provide direct supervision and oversight of clinical team members, including LPNs/LVNs and home health aides
  • Participate in a multidisciplinary team approach, coordinating care with PT, OT, ST, MSW, HHAs, and office staff
