Our dedicated team of healthcare professionals is committed to delivering personalized, high-quality support tailored to meet the unique needs of each individual we serve Hometown Home Health is seeking a dynamic individual to join our team in a Home Health Registered Nurse role. This position plays a pivotal role in maintaining top tier patient care.

Requirements

• Must have a nursing degree from an accredited program
• Licensed as a registered nurse within the state of Florida
• Adherence to federal, state, and accreditation requirements, including Medicare and Medicaid regulations.
• CPR certification required (Online certification not accepted).
• At least one year of experience at a home health agency or within a hospital.

Responsibilities

• Coordinate patient care in accordance with the treatment plan created by the physician.
• Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
• Participate in the patient's discharge planning process.
• Prepare progress notes following visits for patients’ charts.
