About The Position

DaVita Kidney Care is launching a new team, DaVita Skilled Nursing Dialysis Services (DSD Services), to provide industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF). This initiative aims to reduce the burden on approximately 10% of dialysis patients who reside in SNFs and are currently transported for treatment, thereby also reducing avoidable healthcare costs. Teammates joining DSD Services will leverage DaVita's resources to develop and grow this new service line. Ideal candidates will have dialysis experience, thrive in a start-up environment, be passionate about improving patient lives, and demonstrate a proactive approach to getting tasks done. Autonomy and collaboration with cross-functional teams are essential. In this role, the Registered Nurse (RN) is responsible for the complete ongoing care of each patient, ensuring their dialysis treatment progresses as planned. DSD RNs will troubleshoot issues, make critical decisions regarding patient dialysis treatment, and coordinate care with SNF staff. The primary work schedule is three days a week, with occasional Saturday work, and no on-call duty is required. This position offers a significant opportunity to positively impact the lives of patients and their families daily.

Requirements

  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 12 months' of RN experience
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing
  • Bachelor of Science in Nursing (B.S.N)
  • Three-year diploma from accredited diploma program (may be substituted for nursing degree)
  • Basic computer skills and proficiency in MS Word and Outlook

Nice To Haves

  • Previous Chronic Hemodialysis, Acute Dialysis and/or Peritoneal Dialysis (PD) nursing experience
  • Training/Teaching/Healthcare Education background
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment
  • Occasionally partner alongside the SNF staff to coordinate care
  • Conduct impactful and meaningful education to patients and their families in the SNF setting
  • Independently demonstrate best practice techniques and deliver optimal dialysis care
  • Supervise Dialysis Technicians tasked with delivering tandem care to meet capacity and efficiency goals of the program
  • Meet and/or exceed clinical outcomes, with multitasking expected to meet the needs of the service, patients, and SNF partners
  • Provide comprehensive patient care in an independent setting
  • Apply core nursing skills to assess, diagnose, and implement complete care plans by looking at the whole patient
