  • Conduct comprehensive In-home assessments & reassessments
  • Create/update care plans
  • Provide skilled nursing interventions & medication administration
  • Monitor & document client health changes
  • Communicate with clients, caregivers, and healthcare teams
  • Coordinate referrals to healthcare or community resources
  • Maintain accurate documentation per state and agency regulations
  • Participate in caregiver training and in-service programs
  • Current Maryland RN license (or multi-state compact license)
  • CPR/First Aid Certification
  • Reliable transportation for home visits
  • Willingness to work occasional evenings/weekends
  • Knowledge of infection control and patient safety procedures
  • Experience in Home Health, Hospice, or Geriatric Nursing preferred
  • Strong assessment & clinical decision-making skills
