Scope of Work: Registered Nurse on Flourish Health Clinic Key Responsibilities: Flourish Health Clinical Service Delivery The Registered Nurse for the Flourish Health Clinic provides trauma-informed, field-based nursing and medical support to individuals impacted by substance use, mental health conditions, housing insecurity, and barriers to care. As part of the interdisciplinary team, the nurse conducts patient intake, screenings, and triage; performs vital signs assessment, point-of-care testing, immunizations, wound care, and chronic disease monitoring; assists providers with patient evaluations, procedures, and care coordination; and supports acute care and emergency response in alignment with harm reduction principles. The nurse provides harm reduction education and resources, offers brief supportive interventions and crisis-informed care (including de-escalation support), and coordinates referrals and follow-up planning to medical, behavioral health, recovery, and social services. This role also supports inventory management of medical supplies and medications, ensures accurate documentation of assessments, interventions, and referrals in compliance with legal and regulatory requirements, and contributes to student training and continuity of care through collaboration with community partners and the broader healthcare network. We established our non-profit agency by merging three community organizations: Urban Case Management of Meharry Community Health Center, Continuous Treatment Team of Council of Community Services, and Mobile Crisis Team of the Crisis Intervention Center. This fusion resulted in an innovative, comprehensive, and multidisciplinary approach that addresses the specialized needs of individuals living with severe behavioral health conditions and challenges related to poverty. Responsibilities
