About The Position

The Registered Nurse will be responsible for conducting in-person supervisory visits with EPSDT and VA clients. This role includes managing a caseload of approximately 50–70 clients and ensuring that each client is visited in their home every 60 days. This position is non-clinical in nature and does not involve skilled nursing tasks; instead, it focuses on oversight and client well-being. The position serves clients located in the southwest quadrant of Albuquerque, specifically those residing south of I-40 and west of I-25. The expected commitment is approximately 50–60 hours per month.

Requirements

Current and valid Registered Nurse (RN) license in NM.
Must reside in Albuquerque or surrounding areas
Minimum of 1 year of nursing experience, preferably in a home health or community health setting.
Strong assessment and documentation skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage a flexible schedule.
Compassionate and patient-centered approach to care.
Valid driver's license and reliable transportation.
Must be able to work evenings and have weekend availability for the remainder of the school year.

Responsibilities

Conduct face-to-face supervisory visits with EPSDT and VA clients every 60 days.
Ensure compliance with all regulatory and organizational standards during visits.
Assess the overall well-being of clients and provide necessary support and guidance.
Document visit findings and communicate any concerns or recommendations to the appropriate team members.
Collaborate with caregivers and other healthcare professionals to ensure coordinated and effective care.
Maintain accurate and up-to-date client records.
Provide education and resources to clients and their families as needed.
Adhere to all company policies and procedures.
