Job Overview The Registered Nurse is responsible for overseeing weekly medication distribution, addressing medical needs with the ARP provider, and maintaining accurate health records. This role supports medication delivery and collaborates with the ARP team to ensure long-term consumer care and wellness. The RN conducts assessments, monitors side effects, and provides education using evidence-based practices such as psychoeducation, motivational interviewing, and relapse prevention. Additional duties include attending daily team meetings, documenting reports accurately, and connecting patients and families with community resources. Program Info As residential health care providers, we are committed to enriching our individuals’ lives by promoting a helpful and supportive environment. Our intent is to support and assist the individuals in achieving their desired level of recovery within their desired wellness domains. In addition to recovery orientation, the goal of the residential program is to provide skills teaching, education, encourage personal growth and increased demonstration of independence within activities of daily living so the resident can integrate into the community to a residence of their choice. While they remain in the residential program, the goal is to provide the least restrictive but most clinically appropriate structured setting. Our combined efforts will promote physical and mental health, and increase social support identification and engagement resulting in a dramatic decrease in the need for further psychiatric hospitalization.

Requirements

  • NJ Registered Nurse license in good standing.
  • Minimum of one year of experience with individuals with serious and persistent mental illness.
  • Strong work ethic; comfortable working independently.
  • Must demonstrate ability in working with a variety of internal and external departments
  • Must possess knowledge of data reporting and use of an electronic health record.
  • Must demonstrate advanced communication, analytical, and organizational skills.
  • Must demonstrate strong project management skills
  • Must have and maintain a valid driver's license with an acceptable driving record
  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Responsibilities

  • Review weekly medication distribution, review any medical needs and address with ARP provider and manage record keeping.
  • Support medication delivery and collaboration with ARP team to provide long term support to the consumer.
  • Assess consumer’s concerns, mental illness symptoms, and behavior in response to medication and monitor side effects during ongoing face to face contacts.
  • Attend daily morning meetings to address consumer and program needs.
  • Utilization of evident based tools including psycho-education, social skills training, cognitive-behavioral techniques, motivational interviewing, and relapse prevention planning.
  • Educate patient and family/caregiver(s) about relevant community resources
  • Completes and submits accurate reports and chart documentation in a complete and timely manner according to program guidelines and established procedures.
  • Ensures quality and accuracy of written and verbal communications.
  • Other duties as required
