About The Position
Join our Nursing Primary Care Teams The Community Health Center of Cape Cod offers a rewarding, team centered environment. You will be responsible for the coordination of services and pursuit of excellence in integrated care for our patients. Focus on triaging calls and in basket messages, patient education, assessments, implementing and monitoring care plans and ensuring Quality outcomes. Requirements
- Graduate of accredited school of Nursing
- Demonstrated proficiency with EHR
- with EPIC a strong plus
Nice To Haves
- BSN strongly preferred
- worked in an outpatient setting
- experience working with underserved populations
