About The Position

The Registered Nurse (RN) utilizes sound medical and professional judgment and best practices to ensure the medical, safety, and well-being of residents, while adhering to state nursing and licensing regulations and federal standards. Working closely with their assigned nurse manager, the RN is responsible for the care, oversight, and quality assurance of medical care coordination for residents. This includes, but is not limited to, managing well visits, immunizations, sick visits, documentation, medication audits, supply inventory, ordering, and communication with parents/guardians. Breckenridge Village is a caring, faith-based residential community located on a tranquil 80-acre campus west of Tyler, Texas. It serves adults with mild to moderate intellectual disabilities, offering exceptional residential and day enrichment programs. The village is dedicated to providing a warm, family-like setting that empowers residents in their spiritual, physical, mental, emotional, and social development within a safe and loving environment.

Requirements

  • Graduate from accredited Registered Nursing Program.
  • Maintain current good standing as a Licensed Vocational Nurse or Registered Nurse with the state of Texas Board of Nursing.
  • Current BLS certifications and First Aid Certification.
  • Pass a pre-employment drug screen and random drug screens throughout employment.
  • Valid Texas Driver’s License.
  • Pass a pre-employment and biennial criminal background checks.
  • Must be able to maintain professional people skills, oral, written, listening, and non-verbal communication skills.
  • Demonstrate the ability to respond sensitively and competently to the service population’s cultural and socio-economic characteristics.
  • Demonstrate the ability to work collaboratively with other staff members, service providers, and professionals.
  • Demonstrate the ability to communicate effectively in writing and verbally in English.
  • Demonstrate the ability to work in a fast paced environment while maintaining control and professional composure at all times and making decisions based on the needs of the program and service population.
  • Demonstrate the ability to maintain computer literacy required to meet the responsibilities of the position.

Responsibilities

  • Perform nursing duties in accordance with nursing guidelines and best practices established by the Texas Board of Nursing for Registered Nurses.
  • Perform quality assurance oversight and regular auditing of T-logs and GERS daily.
  • Perform quality assurance oversight and regular auditing of immunizations.
  • Perform quality assurance oversight and regular auditing of medication ordering and delivery.
  • Perform quality assurance oversight and regular auditing of distribution of medications.
  • Perform quality assurance oversight and regular auditing of medication supply.
  • Perform quality assurance oversight and regular auditing of medication storage, quarterly assessments, and destruction.
  • Perform weekly audits of MAR.
  • Respond to injuries and illness during normal business hours.
  • Maintain real time medical documentation.
  • Create nurse progress note, in real time, in PCC when any communication regarding medical care is received from or provided to a resident, guardian, or Breckenridge Village staff member.
  • Review appointment summaries and audit documentation into PCC.
  • Audit medication administration records (MAR) in real time when medications are changed, added or discontinued.
  • One week prior to each scheduled Human Rights Committee (HRC) meeting, obtain and complete all HRC approvals and review forms and submit to Nurse Manager.
  • Participate in on-call rotation with LVN/RNs, and Nurse Manager 24/7 for Nurse on Duty (NOD) coverage to Breckenridge Village.
  • Respond to all calls immediately, but no later than 10 minutes after call is received, during NOD rotations.
  • Be available to respond to the medical needs of residents on campus as soon as possible during NOD rotations.
  • Have contact information readily available in the event notification is required during NOD rotations.
  • Prepare and distribute written summary of all weekend communications no later than noon on Monday.
  • Attend in-person or video conferencing meetings as directed, including Annual IDT, Bi-annual HRC, Off-cycle HRC, and Weekly consultation with Nurse Manager.
  • Attend weekday/end manager meetings, weekly medical meetings and all staff meetings.
  • Conduct annual comprehensive medical exams.
  • Audit quarterly assessments.
  • Provide necessary training for medical staff and medical extender staff or other BV staff.
  • Conduct doctor follow up on abnormal exams or specialty requests.
  • Pull and review special procedure diagnostics.
  • Conduct 30 day staffing and medication review for new residents.
  • Audit labs.
  • Audit follow up appointments.
  • Audit Contact Sheet Fulfillment.
  • Audit Physician orders for changes, THV, MAR, Contact Sheets.
  • Audit Cancelled/rescheduled appointments.
  • Audit Vitals and weights.
  • Maintain professional setting, organized supplies and charts and maintain expiration dates for DME and CMS.
  • Report signs of abuse, neglect, and exploitation and sexually provocative behavior and implement professional response, reporting and documenting procedures.
  • Adhere to professional and ethical standards of conduct outlined in Breckenridge Village personnel polity manual including demonstrating respect for agency staff, residents, and community members and complying with required dress code at all times.
  • Meet all state regulatory guidelines and standards that are applicable to this position, with no deficiencies or concerns regarding medical care during annual fundamental surveys.
  • Maintain resident physical and mental safety by making immediate report of any changes in resident health of behavior to the Nurse Manager and the QIDP with whom resident is assigned.
  • Provide the highest level of resident care, including daily review of T-logs and GERS, daily review of communication, addressing each medical concern with urgency, treating resident medical needs as though for own family, and calling 911 first in emergency situations.
  • Ensure proper time management of all daily, weekly, monthly, quarterly, and annual tasks.
  • Document all resident health information as close to real time as possible but not later than 24 hours.
  • Answer all phone calls immediately or call back within one hour during normal business hours.
  • Answer text messages immediately or text back within one hour during normal business hours.
  • Answer all emails within the same business day during normal business hours.
