About The Position

We are seeking a dedicated and compassionate Registered Nurse (RN) to provide skilled nursing services and health oversight for individuals in the West Virginia Medicaid Waiver program (I/DD). Key Responsibilities Assessment & Care Planning: Conduct initial and ongoing nursing assessments (quarterly/annual reviews) to determine medical eligibility and individual health needs. Develop, evaluate, and revise individualized care plans in collaboration with the interdisciplinary team (IDT), physicians, families, and direct care staff. Medical Oversight & Case Management Support: Monitor the health status of individuals, document changes, and communicate with physicians and other healthcare professionals. Ensure all physician orders for medications and treatments are current, properly documented, and implemented. Training & Education: Provide health-related training and guidance to unlicensed personnel (e.g., LPNs, AMAPs, Direct Support Professionals) on an individual's specific medical needs and interventions. Documentation & Compliance: Maintain accurate and up-to-date health records, medication administration records (MARs), and other required documentation in compliance with WV Bureau for Medical Services and waiver program regulations. On-Call Support: Participate in an on-call rotation to provide nursing support

Requirements

  • Graduate of an accredited school of nursing.
  • Active, unencumbered Registered Nurse (RN) license to practice in the state of West Virginia or a compact state license.
  • Minimum of two (2) years of nursing experience (preferred).
  • Must have a valid driver's license, reliable transportation, and valid auto insurance
  • Ability to perform tasks required to support individuals in the program and maintain a safe environment.
  • Successful completion of required background checks
  • Strong medical assessment, organizational, and communication (written and oral) skills.
  • Proficient in computer skills for documentation and electronic systems.

Nice To Haves

  • Experience working with individuals with intellectual/developmental disabilities or the aging population (preferred).
  • Experience in home healthcare or community-based settings is highly desirable.
