POSITION SUMMARY: The RN will provide skilled nursing services to both opioid dependent patients as well as those individuals who are suffering from Serious Persistent Mental Illnesses (SPMI). The RN must understand the needs of patients and assist them with overcoming addictions and mental health disorders that also impact patients' families. The RN provides treatment to patients and ensures medications are correctly administered. The RN also collaborates with team members within Community for New Direction, outside referral sites, and with providers and staff from other healthcare organizations throughout the county and state. The RN must understand patient care goals as voiced by the patient, and they must make the patient a major part of his/her treatment and recovery while providing advocacy as needed to ensure the patient knows and understands his/her rights. The RN also performs ongoing assessments, intake, evaluations, triage, teaching, inductions, stabilizations, and monitoring of patients in medication assisted treatment programs. ESSENTIAL FUNCTIONS: In collaboration with the Behavioral Health Clinician (BHC), performs initial assessments and intake obtaining social, medical, mental health, substance abuse, and medication history. Collaborates, communicates, and meets with MAT treatment team as needed and required. Assess patients by discussing their mental health issues and/or addictions to draw up the best possible ways to care for them. Builds a robust and excellent relationship with patients and their families by cultivating trust and monitoring patients’ progress with their families. Ensures the coordination of all related mental health services by conducting risk assessments on patients. Provides education and assessments to patients looking to access MAT treatment programs. Assesses and monitors patients in the induction, stabilization, and maintenance phases of treatment. Provides ongoing management, education, and support of patients in all phases of treatment including nursing visits scheduled and random, urine-toxicology screens, routine labs, and medication teaching, monitoring, pill counts, and medication refills as needed. Exhibits non-judgmental, empathetic, and supportive approach when communicating with patients/families and staff. Follows state and federal guidelines while providing care to opioid dependent patients in collaboration with licensed, prescribing physicians and pharmacies. Provides treatment to patients to ensure medications are correctly administrated or recommend patients to psychiatrists and psychologists for further examination. Assists patients in overcoming their addictions and mental health problems and coping or managing their emotions effectively. Provides education regarding addictions and mental illnesses, physical health concerns, chronic disease management, wellness, relapse prevention and medications to patients, families, care givers, and team members. Completes appropriate documentation with record keeping. Assists in the ongoing primary care of patients referred to the opioid treatment program. Consistently uses clear, concise, and effective written and oral communication; and complies with departmental policies when sharing/documenting relevant patient care data. Works with the pharmacies and the MAT team in obtaining prescriptions, refills, and prior authorizations as needed. Documents all calls/visits; maintains accurate notes on telephone messages and attaches them to progress notes in patients' charts and in a patients' medical records. Records must be complete and easily understood by anyone who may need to access the record. In cases of patient referral to the emergency department or another facility, coordinates care with appropriate staff at the other facility (Mental Health Center, Emergency Department, other Community-Based Substance Abuse and Mental Health Treatment facilities etc.). Assists with non-scheduled nursing needs during/throughout the shift. These requests vary from assistance with walk-ins, triage, stabilizing an acutely ill patient until the ambulance arrives, or other stabilizing interventions. Actively participates in policy/protocol development for the program. Attends annual mandatory in-services as designated by CARF, OHIOMHAS (CPR/First Aid, De-escalation, and trainings). Participates in staff meetings, committees, attend professional development conferences, trainings, webinars/seminars as needed to as enhance your knowledge, broaden your horizon and obtain continued education. Performs other duties as assigned to promote the overall mission and vision of Community for New Direction.
