About The Position

The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational. Stephen Russo Residence (W 107th Street), is a permanent supportive housing residence for formerly homeless adults with mental health and co-occurring disorders. Services include case management, harm reduction, housing readiness, counseling, medical and psychiatric services, groups and recreation, and 24-hour 7 day a week front desk concierge. The Registered Nurse is responsible for delivering direct and comprehensive nursing care for Senate tenants. The Registered Nurse will support the coordination of medical care and psychiatric care to increase positive health outcomes for tenants.

Requirements

  • NYS RN license required.
  • Knowledge of working with older adults, adults with mental illness and substance use concerns.
  • Understanding of harm reduction and trauma informed care.
  • Ability to multitask in a fast-paced environment.
  • Strong verbal and written communication skills.
  • Ability to utilize electronic health record databases.
  • Ability to work in Microsoft Office Suite, proficient in Microsoft Outlook, Excel, and Teams.
  • Able to use or learn to use AWARDS.
  • Sitting for an extended period of time standing and walking
  • Visit clients in their apartments in The Senate Residence
  • Able to lift up to 15 lbs.

Nice To Haves

  • BSN preferred.
  • Spanish speaking preferred.

Responsibilities

  • Administer medications, injections, and treatments.
  • Monitor, pack and dispense medication to clients. Keep record of client medication schedules.
  • Documents patient care services by charting in patient records and supporting data tracking and analysis.
  • Provide wound care.
  • Collaborate with on-site NP, community physicians, psychiatrists and case managers to ensure optimal treatment plan for clients.
  • Coordination of care with tenants’ providers and local pharmacy.
  • Provide education to clients regarding chronic illness management and prevention.
  • Assess acutely ill clients and determine appropriateness of emergency services.
  • Conduct home visits to provide in-home care for clients who are home-bound.
  • Communicate with social service staff any medication or health changes.
  • Track and maintain medical supply inventory.
  • Conduct biweekly wellness & smoking cessation groups.
