About The Position
TLC HomeCare Inc. is currently seeking a dedicated and experienced Registered Nurse (RN) to perform in-home patient assessments. As a family-owned and operated incorporation, we make sure that every member of our team represents our values. Having over 20 years of private home care experience, we know just how important your loved ones are. We understand that taking proper care of them can be challenging which is why we administer the highest level of care for them possible by placing only certified care staff in their homes. We take the time to know and understand our clients personally, which allows us to match them with the perfect caregiver.
Requirements
- Active RN license
- Experience in home care or clinical assessment preferred
- Strong communication and organizational skills
- Reliable transportation and willingness to travel
Responsibilities
- Conduct initial and ongoing patient assessments in clients’ homes
- Develop and review care plans based on patient needs
- Coordinate with caregivers and healthcare professionals
- Ensure accurate and timely documentation
