About The Position

The Registered Nurse (RN) at the Ambulatory Surgery Center is a critical member of the surgical team, responsible for providing comprehensive nursing care across all perioperative phases: Pre-Operative (Admitting), Intra-Operative (Operating Room), and Post-Anesthesia Care Unit (PACU). This role ensures patient safety, comfort, and optimal outcomes through thorough assessment, planning, implementation, and evaluation of care. Key responsibilities include: Conducting pre-operative assessments, initiating IV therapy, administering medications, and educating patients and families about procedures and rights. Delivering intra-operative care by maintaining aseptic techniques, assisting with anesthesia, monitoring patient status, and ensuring surgical safety protocols are followed. Managing post-operative recovery by monitoring airway, vital signs, pain control, and readiness for discharge, while responding promptly to emergencies. The RN collaborates with physicians, anesthesia providers, and other healthcare professionals to maintain a high standard of care, uphold patient rights, and ensure compliance with hospital policies and regulatory standards.

Requirements

  • Graduate from an accredited school of nursing.
  • Licensure as a Registered Nurse in the State of Idaho required.
  • Current BLS, ACLS, and PALS certification required.
  • Operating room experience preferred or one year nursing experience.
  • Operating room equipment and computer knowledge.
  • Must have the ability to effectively read, write, and speak English.
  • Dosage Calculation and General mathematical Skills
  • Ability to work in a fast-paced environment.
  • Can adapt to rapidly changing workloads due to changes in the surgery census.
  • Can frequently and consistently make sound judgments.
  • Works well with others.

Responsibilities

  • Adheres to and practices MVH Compliance Program and participates in Performance Improvement activities.
  • Is knowledgeable of policies and procedures of the department and facility.
  • Maintains professional competency through appropriate continuing education.
  • Performs other related duties incidental to the work described herein, or as assigned by the Team Leader/Charge Nurse, OR Manager, and Director of Surgical Services.
  • Demonstrates an awareness of safe conditions and safe acts as related to hospital employment.
  • Maintain BLS, ACLS and PALS Certifications
  • Complies with attendance requirements per Mountain View Hospital Policy.
  • Performs the perioperative nursing role to include preoperative assessment of the patient, nursing care plan formulation, operative care plan implementation, and postoperative evaluation of the patient’s response/outcome. Also evaluates the effectiveness and appropriateness of nursing care provided with documentation of such.
  • Demonstrates understanding and utilization of principles of asepsis in all duties.
  • Assists in the setting up and performance of cases by assembly of equipment and supplies according to sterile technique.
  • Ensure history and physical is signed by physician and within 30 days of surgery. Reads time out from the surgical consent prior to start of the procedure.
  • Assists in the setting up and performance of cases by opening of sterile pack, instruments, and supplies according to sterile technique.
  • Prepares each patient for surgery by: positioning the patient in the appropriate position, preparing the patient’s operative site, confirming the procedure and site according to the surgeon’s history and physical and operating room team, and helping move the patient to and from the operating table. Obtain ordered medications through medication dispensing system. Prepare ordered solutions for surgical cases.
  • Assists anesthesia personnel with the intubation/extubation of patients, the monitoring of fluid volume balance, the administration of blood/blood products, and the thermo-regulation of the patient.
  • Performs and documents surgical counts (sponges, sharps, and instruments) and documents nursing care on legal records.
  • Handles and logs in specimens according to proper procedure/policy. Orders lab supplies as needed.
  • Assists in the clean up of the operating room following surgery, including proper disposal of used supplies and sharps.
  • Assumes responsibility for intra-operative patient safety related to positioning activities, the use of equipment, and the use of chemicals and solutions.
  • Is responsible for complete and thorough performance of assigned tasks, addressing specific needs of pediatric, adolescent, adult and geriatric patients as necessary.
  • Acts as the nursing advocate for the patient, upholding the patient’s rights. Collaborates with other professional team members.
  • Monitors the patient’s level of consciousness, electrocardiogram, blood pressure, and oxygen saturation during local anesthetic procedures.
  • Uses supplies conservatively and efficiently.
  • Performs infection control activities to include observation of the surgical team during procedures, taking corrective actions as needed to maintain aseptic technique. Provides a surgically clean environment for patient care. Ensures the sterility of supplies and instrumentation used during operative procedures.
  • Maintains area in a state of readiness at all times, ensuring that all necessary equipment is available and in good working order, with age specific supplies ready for use as needed.
  • Assesses physical and emotional needs of patients and implements appropriate nursing interventions. Completes the nursing pre-assessment record, including confirmation of the following: a. Allergies. b. Medications taken at home. c. Dentures, contact lenses and glasses. d. Pertinent medical conditions listed. e. Pertinent past medical history listed. f. Confirmations of consent and operative site. g. Confirmations of History and Physical within 30 days of surgery. h. Confirmation of responsible party. i. Confirmation of ordered labs/test results on chart.
  • Respects the patient’s right to privacy, dignity and confidentiality.
  • Assists the patient in undressing if needed and stores belongings appropriately.
  • Assesses or performs the following: a. Vital signs. b. Weight and height. c. Accu-Check for blood sugar, if applicable. d. Pregnancy test, if applicable. e. Starts IV on all patients, except those who have an order not to start an IV. f. Utilizes anesthesia protocol regarding IV fluids and catheter size. g. Pain level. h. Clipping patient if necessary.
  • Performs pre-operative instruction and explanation of procedures and patient’s rights.
  • Gives appropriate preoperative medications per orders.
  • Ensures that patients are seen by the anesthesia provider prior to being taken to the O.R. Ensures device reps are there and met with patient prior to patient being taken to surgery.
  • Sets priorities to facilitate the timely and efficient flow of patents throughout the day.
  • Carries out physician/anesthesia orders.
  • Records pertinent patient information on the nurse’s notes.
  • Demonstrates an economic use of equipment and supplies.
  • Maintains confidentiality of all pertinent patient care information to assure patient’s rights are protected.
  • Demonstrates commitment to excellence in patient, family and physician relations.
  • Communicates appropriate information to other members of the healthcare team in a timely manner.
  • Acts as the nursing advocate for the patient by upholding patient rights.
  • Participates in overall Pre-Op/PACU Department functions, including, but not limited to: a. Follows appropriate Pre-Op/Admitting policies/procedures. b. Training new staff as needed. c. Attends and participates in staff meetings. d. Attends annual education in-services as required. e. Is knowledgeable about and responsive to emergency situations, i.e. cardiac arrest, malignant hyperthermia, drug reaction, fire, disaster, etc.
  • Complete preoperative calls per policy.
  • At the end of shift: a. Checks with PACU i.e. staffing needs, before leaving for the day. b. Stocks the blanket warmer. c. Prepares IV’s/tubing for the next working day. d. Readies rooms: gurneys made up, O2 tanks >500psi. e. Restocks IV trays. Restocks room carts.
  • Maintains the PACU area in a state of readiness at all times, ensuring that all necessary equipment is available and in good working order, with age specific supplies ready for use as needed.
  • Assess patients on admission to PACU for the following: a. Patient airway b. Respirations c. Vital Signs d. Level of consciousness according to the Aldrete scale e. Level of pain
  • Assesses and records vital signs every 5 minutes for the first 20 minutes until awake, 15 minutes x2, then every 30 minutes and documents Aldrete scale every 15 minutes until patient meets criteria of PACU 2. Records pain scores every 15 minutes and documents discharge vital signs to include temperature.
  • Uses appropriate devices and techniques to assist patient’s respirations and maintains optimum blood oxygen saturation.
  • Assess surgical site for bleeding, reinforce dressing and notifying physician if increased drainage.
  • Able to assess and treat post-operative pain effectively
  • Proficient knowledge of drugs used in the PACU.
  • Positions patient as ordered by physician.
  • Assesses and records intake and output of all fluids.
  • Carries out physician / anesthesiologist orders.
  • Assesses patient for transfer to PACU 2 when Aldrete scale is at “8” with no “0” in any category.
  • Responds appropriately to emergency situations, recognizing the need for assistance and obtaining support from anesthesia personnel and/or physician as needed.
  • Preforms discharge instructions or appropriate report to staff.
  • Transfers patients to Medical Surgical unit when applicable.
  • Records pertinent patient information on nurse’s notes.
  • Demonstrates economic use of equipment and supplies.
  • Maintains confidentiality of all pertinent patient care information to assure the patient’s rights are protected.
  • Demonstrates commitment to excellence in patient, family and physician relations.
  • Acts as the nursing advocate for the patient, upholding the patient’s rights.
  • Communicates appropriate information to other members of the health care team in a timely manner.
  • Participates in overall Pre-Op/PACU Department functions including, but not limited to: a. Follows appropriate PACU policies/procedures. b. Training new staff as needed c. Attends and participates in staff meetings d. Attends annual educational in-services as required e. Is knowledgeable about and responsive to emergency situations, i.e. cardiac arrest, malignant hyperthermia, drug reactions, fire, disaster, etc.
  • Stocks medication dispensing system with new stock.
  • End of Shift a. Stocks the blanket warmer. b. Restocks IV trays. Restocks room carts. c. Readies rooms: gurneys made up, O2 tanks >500psi. d. Re-verifies narcotic count/secures prescription pads e. Locks drug cupboards and supply cart f. Locks facility door upon exiting.
  • Make copies of the surgery schedule for RN’s and anesthesia providers.
  • Reviews and updates the surgery schedule and staffing if needed.
  • Be available by the surgery phone for questions and concerns.
  • Coordinates the flow of patients and assignments with nurse manager.
  • Supervises and trouble-shoots the operations of the department for the day.
  • Checks with PACU i.e. staffing needs
  • Is knowledgeable about and responsive to emergency situations, i.e. cardiac arrest, malignant hyperthermia, drug reaction, fire, disaster, etc.
