About The Position

Seeking Private Duty Licensed Vocational Nurses and Registered Nurses to join our team of nurses who are committed to the highest standard of care, integrity and reliability. Hallway Healthcare provides in-home nursing care to retired Nuclear and Federal Workers that have received benefits through EEOICPA or FECA. These are the workers who have retired from Department of Energy facilities such Lawrence Livermore National Lab, Sandia National Lab Lawrence Berkeley National Lab, Stanford Linear, etc. We also serve Federal Workers such as US Postal Workers. Requirements

  • Graduated from an accredited school of professional nursing.
  • Current California RN/LVN License
  • Current BLS Certificate

Responsibilities

  • Conduct initial home care patient visits and regularly reassess patient needs and progress.
  • Develop and initiate the plan of care in accordance with the physician’s orders.
  • Perform comprehensive assessments of home care patients.
  • Observe, evaluate, and document symptoms accurately.
  • Monitor patient reactions and progress throughout treatment.
  • Educate patients and caregivers about disease processes, medications, the plan of care, and individualized treatment strategies.
  • Provide instruction to patients and caregivers on techniques for in-home health care.
  • Coordinate and facilitate patient services efficiently.
  • Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
  • Communicate any changes in the patient’s condition to the physician and relevant team members (DPC, PT, Nurse Supervisor).
  • Follow the agency’s approved policies and procedures when performing skills.
  • Discharge patients from skilled nursing services once discharge criteria are met.
  • Collaborate with clinicians during case conferences to ensure coordinated care.
  • Maintain and update clinical documentation in accordance with policies and procedures. Documentation must be completed by the end of each shift. If unable to do so you must inform your Case Manager.
  • Continuously enhance professional knowledge and skills through attendance at in-service trainings, continuing education, seminars, and self-study annually.
  • Provide onsite supervision of LPNs and HHAs.
  • Comply with all applicable state regulations.
