About The Position

The Registered Nurse is responsible for providing safe and efficient care to patients in an outpatient community health center. The Registered Nurse serves as a member of a multidisciplinary care team providing primary and urgent care to a multicultural and diverse patient population, performs professional nursing duties and maintains high standards of patient care.

Requirements

  • Requires completion of high school and graduation from an accredited school of nursing. BSN preferred.
  • Possession of a valid temporary/permanent California Registered Nurse license.
  • Active BLS Certification required.
  • Requires thorough working knowledge of modern methods, techniques and procedures used in professional nursing, including understanding of medical terminology and clinic routines, procedures, equipment and facilities.
  • Requires the ability to: work cooperatively with others, keep patient case records and prepare related reports; plan, organize, assign and inspect the work of sub-professional subordinates; use good judgment in unusual and emergent situations.
  • Excellent interpersonal, listening, oral, and written communication skills are required.
  • Familiarity with computers/information systems (e.g., EHR, Microsoft office programs). Ability or willingness to learn to use information and communication technologies (computers, smartphones, and iPads) to find, evaluate, create, and communicate information.
  • Knowledge of public health principles and population-based programs.
  • Experience with and/or knowledge of health and environmental issues in the Latino, Asian, LGBTQ, HIV/AIDS and homeless communities.
  • Experience in working with a pediatric population and Vaccines for Children program
  • Strong personal interaction skills including skills in negotiation and conflict management.
  • Commitment to St. Anthony Foundation Statement of Values and a desire to work for a social services agency serving the poor.

Nice To Haves

  • Prefer one year of experience, preferably in Med-Surgical, Ambulatory care and/or Community Health.
  • Proficiency in a second language is strongly desired (e.g. Spanish, Mandarin, Tagalog, Cantonese, Arabic).
  • Experience in ambulatory primary care practice management preferred.
  • Experience in working collaboratively with community-based organizations.

Responsibilities

  • Performs initial assessment and measurement of vital signs of patients presenting to clinic with urgent/same day visits in a timely manner to determine priority and expedite care if clinically indicated.
  • Provides clinical support to providers and participates as part of the health care team to assure coordination and continuity of patient care.
  • Provides team-based medical management in collaboration with the patient’s primary care team for patients with chronic disease.
  • Performs nursing assessment, implements plan of care and arranges for follow up care for primary care patients including adults and children
  • Perform medication reviews, blood pressure and blood sugar checks, and wound care.
  • Provide patient education on a variety of health topics (high blood pressure, heart disease, nutrition, diabetes and specific diagnostic procedures).
  • Responds to emergencies as they occur in a timely manner with clear delegation to medical assistant staff and reports to medical provider staff.
  • Delegates tasks to medical assistants in order to enhance patient care and clinic flow, and mentor medical assistants or volunteer staff.
  • Administers immunizations (pediatric/adult), medications and blood draws consistent with current guidelines and established protocols.
  • Have flexibility in meeting patients’ needs, and participating in evening and Saturday clinics.
  • Collaborating with internal and external medical, psychiatric, substance use management and social service agencies to facilitate entry or services to the pt.; completion of periodic home visits to assess the patient and environment;.
  • Participates in the development and implementation of educational programs and materials for patients and their family members.
  • Facilitate and support medical providers with procedures.
  • Works with other members of the health care team to provide comprehensive primary care services.
  • Treat all guests with empathy, dignity, and respect. Seek to understand the other person's perspective and experience.
  • Demonstrate the ability to work effectively across cultures and an openness to learning about cultures not familiar to oneself.
  • Complete reports and all other documentation in according with professional standards.
  • Enter data and information electronically as required. Looks up data and reports electronically through a database as requested.
  • Understand and follow the organization's programs, policies, and procedures
  • Participate in regular training and development opportunities to increase personal capacity to apply innovative approaches to daily workflows, service delivery, and guest care and engagement.
  • Routinely checks for communications via telephone, electronic or fax during the course of a clinical day, consults with clinical staff as needed and responds appropriately.
  • Utilizes the electronic health record to document all patient encounters and communications pertaining to the patient’s care.
  • Uses multiple EHR platforms to gather patient information to assist with patient care planning.
  • Provides panel management of patient cohorts with abnormal screening results.
  • Evaluate work flow and activities to assure efficiency and quality health care to all patients.
  • Communicates effectively with the client/patient, clinic staff, the public at large and departmental staff in a non-discriminatory manner.
  • Participates in daily quality improvement (QI) activities.
  • Train and act as a resource to non- licensed staff working in the clinic (e.g. Medical Assistants, volunteers, nursing students).
  • Participates in pre-clinic daily huddle, identifies high needs patients for the clinic session on an as needed basis.
  • Attends and participates in clinic and community meetings, as is appropriate or required.
  • Participate in relevant meetings supporting team communication and the program's capacity to fulfill organizational goals and mission.
  • Collaboratively works with others to achieve team success.
  • Manage stress and pressure situations calmly and responsively. Maintain the capacity to control reactions and awareness of how their behavior or response can impact others.
