About The Position

Are you a Registered Nurse looking for flexible hours and meaningful work? Summit Home Care & Hospice is excited to expand our services with a new caregiving division—and we’re looking for compassionate, dependable Registered Nurse (RN) to grow with us! In this role, you’ll provide intermittent, non-skilled nursing visits in clients’ homes, supporting both patients and our care team. Registered Nurse (RN): Job Type: PRN/As Needed Location: Richland County & Surrounding Counties (Ashland, Knox, Morrow & Crawford Counties) Pay Rate: $25.00 - $47.00 per Hour (Based on Visit Type) Travel: Required for In-Home Visits

Requirements

  • Graduate from an accredited nursing school.
  • Current licensure in the state of Ohio.
  • Must have current CPR and First-Aid certifications.
  • Possess a valid driver’s license and reliable, insured transportation.

Nice To Haves

  • 1-2 years of nursing experience in a non-skilled home health care setting is preferred.
  • Experience supervising Home Health Aides in a management capacity preferred.
  • Knowledge of Medicaid waiver programs and ODA or PASSPORT programs
  • Experience with electronic visit verification (EVV) systems.
  • Familiarity with non-skilled services and personal care support.
  • Prolonged or considerable walking or standing.
  • Able to lift, position or transfer patients.
  • Able to lift supplies and equipment.
  • A considerable amount of reaching, stooping, bending, kneeling, or crouching may be required.
  • Visual acuity and hearing are required to perform nursing skills.

Responsibilities

  • Perform non-skilled home health RN duties such as supervisory visits, client assessments, and care plan development in accordance with ODH regulations.
  • Conduct initial start of care and assessments for new clients.
  • Supervisory visits for aides (as needed to meet 60-day regulations).
  • Ensure compliance and high-quality care across our personal care clients.
  • Develop or update individualized service plans in collaboration with clients, family members, and caregivers.
  • Provide teaching and training to aides as needed, particularly for changes in client condition or care plan.
  • Document all assessments, visits, communications, and supervisory notes accurately and within 24 hours in the electronic medical record (EMR).
  • Maintain communication with the physician and other care team members as necessary.
  • Report changes in client condition to appropriate parties promptly and take necessary follow-up actions.
  • Monitor compliance with infection control protocols and safety standards.
  • Participate in care conferences, audits, and QA activities as required.
  • Conduct consistent coaching/training sessions with direct reports.
  • Assist with onboarding or training new home health aides as needed.
  • Other duties as assigned by Administrator.
