About The Position

The RN is responsible for conducting comprehensive health assessments, managing medications, and providing nursing support to clients receiving mental health services. Working closely with an interdisciplinary team of therapists, case managers, and psychiatrists, the RN ensures that clients receive individualized, holistic care. The RN is also responsible for educating clients and their families about mental health, medication compliance, and overall wellness, using a patient-centered approach. This role requires a compassionate, adaptable professional who is committed to promoting mental wellness and recovery.

Requirements

  • Active RN license in the state of Georgia.
  • Minimum of 1-2 years of nursing experience, preferably in a behavioral health setting.
  • Experience with medication management and administering psychiatric medications.
  • Strong interpersonal and communication skills with a focus on patient-centered care.
  • Ability to work both independently and as part of an interdisciplinary team.
  • Familiarity with Electronic Health Records (EHR) systems
  • Ability to manage a flexible caseload and adapt to the needs of community-based services.
  • CPR and First Aid certification required

Responsibilities

  • Provide direct nursing care and health assessments to clients, including monitoring vital signs, managing medications, and assisting with treatment plans.
  • Collaborate with psychiatrists, therapists, and case managers to ensure comprehensive care plans are in place for clients.
  • Conduct medication management sessions, including administration, education, and monitoring of side effects for clients.
  • Educate clients and their families on health management, medication compliance, and wellness strategies.
  • Conduct follow-up visits with clients in the community, at home, or via telehealth as needed.
  • Assist in the development of individualized care plans and maintain detailed, accurate documentation of client interactions in the Electronic Health Record (EHR) system (Credible).
  • Monitor and report changes in clients' conditions to the treatment team, including behavioral, physical, and emotional health.
  • Participate in interdisciplinary team meetings to discuss client progress, challenges, and treatment adjustments.
  • Adhere to all CORE program guidelines and ensure compliance with Medicaid regulations.
