Requirements

  • Graduate of an accredited school of professional nursing.
  • Current license as a Registered Nurse in the state(s) of practice.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.
  • Current CPR certification.
  • COVID 19 vaccination required.
  • Strong communication and interpersonal skills.

Nice To Haves

  • BSN, PHN preferred.

Responsibilities

  • Performing comprehensive initial assessments of client status, including physical, psychosocial, and environmental parameters.
  • Developing an individualized plan of care for each client with client input.
  • Providing interventions with a focus on achieving realistic client outcomes.
  • Conducting client reassessments and follow up visits.
  • Maintaining compliant client records showing systematic assessments, planning interventions, and evaluations.
  • Provide training, delegation, supervision and support to our staff of caregivers.
  • Address any issues with respect to clients and or caregivers that may arise.
