About The Position

BAYADA Home Health Care is seeking a Registered Nurse for a Per Diem Weekend SOC (Admissions) opportunity throughout Howard County, Maryland. This position involves servicing clients within senior living communities, offering home health care without the typical extensive commute. As an integral member of a multi-disciplinary health care team, the Registered Nurse will provide rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home. The role requires availability on Friday, Saturday, and Sunday or Saturday and Sunday between 8:30 am and 5:00 pm. Current Home Health and Oasis experience are required, along with one year of prior clinical experience as a licensed Registered Nurse.

Requirements

  • Current Home Health and Oasis experience are required
  • One year prior clinical experience as a licensed Registered Nurse is required
  • A current license as a Registered Nurse in Maryland
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Solid computer skills
  • Ability to travel to cases as assigned

Nice To Haves

  • Prior home care experience strongly preferred
  • Prior experience with electronic medical records (EMR) preferred

Responsibilities

  • Follow a designated care plan in accordance with patient's needs
  • Make home visits to clients in designated geographic territories
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
  • Follow up with, execute, and properly document doctors' orders
  • Perform client assessments as necessary
  • Case management and coordination
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
