About The Position

Integrated Community Services is hiring a Full-Time RN to provide HCS (Home and Community-Based Services) nursing case management to individuals with intellectual disabilities residing in group homes in the north area of Houston (Spring/Tomball area). This position provides a unique and rewarding opportunity to assist individuals in maintaining optimal health and independence in the community. Tasks are similar to home health nursing, with some duties and documentation specific to HCS rules. Schedule (negotiable) : Monday – Friday, 8AM to 4PM, with flexibility to work an alternative schedule when necessary

Requirements

• Bachelor's Degree in nursing Or Associates Degree in nursing
• Current RN licensure with the Board of Nursing for the State of Texas is required
• Current BLS from the American Heart Association is required
• (1) year of professional nursing experience
• Valid Texas Driver’s License
• Clear background check
• Strong communication skills, friendly approach, and team-oriented
• Computer skills -Word, Excel, Outlook, electronic health record (we will train you thoroughly)

Nice To Haves

• Preferred experience in triage and nursing case management
• Related experience with IDD population helpful, but not required.

Responsibilities

• Comprehensive Nursing Assessment
• Pharmacy oversite and medication management
• Coordination of medical appointments and outside care for assigned individuals
• Training and oversight of delegated tasks to direct support staff
• Oversight of medical care, as needed, to promote health, wellness, and occasional recovery from illness or injury
• Service documentation per HCS fee for service guidelines
